Request for Information Number VA791-14-I-0081 Assisted Living - Traumatic Brain Injuries (AL-TBI) 1.0DESCRIPTION Public Law 110-181, the Fiscal Year 2014 National Defense Authorization Act (NDAA), Title XVII, Section 1705 directs the Secretary of Veterans Affairs (VA) in collaboration with the Defense and Veterans Brain Injury Center (DVBIC) of the Department of Defense to implement a five-year pilot program to assess the effectiveness of providing Assisted Living (AL) services to eligible Veterans with Traumatic Brain Injury (TBI) to enhance their rehabilitation, quality of life, and community integration. The Assisted Living Pilot Program for Veterans with Traumatic Brain Injury (AL-TBI) commenced on October 6, 2009. On August 7, 2014, President Obama signed the "Veterans Access, Choice, and Accountability Act of 2014" into law, which amended Section 1705 of Public Law 110-181 to extend the Assisted Living Pilot Program for Veterans with Traumatic Brain Injury (AL-TBI) until October 6, 2017. The VA Denver Acquisition and Logistics Center (VA DALC) in support of the Veterans Health Administration (VHA) is seeking information on the availability and interest of providers of TBI specialized Assisted Living for Traumatic Brain Injury for eligible Veterans with TBI to promote retention and/or improvement in skills related to activities of daily living (ADL) or instrumental activities of daily living (IADL) and the social, behavioral and adaptive skills necessary to enable the Veteran to maintain functional gains and stability or potentially to reside in a non-institutional setting. Services provided will be in accordance with the code of ethics of the appropriate profession. The contracts projected to be awarded would have a Base year, with four (1) year options. Multiple indefinite delivery type contracts are planned to cover several locations within the continental United States. Interested providers are invited to review the attached Statement of Objectives (SOO) and provide comments along with capabilities and capacity to provide the desired services. Specifically, comments are encouraged in the following areas: 1.CARF Accreditation: Does your facility hold a 3 year accreditation in Residential Rehabilitation/Brain Injury Specialty Program - Adults by the Commission on Accreditation of Rehabilitation Facilities (CARF). If not CARF accredited, please explain why your facility is equivalent to one that is CARF accredited in Residential Rehabilitation/Brain Injury Specialty Program. 2.Which services described in the SOO does your facility normally provide? 3.How do you normally bill for the rehabilitation services described in the SOO? Is there a daily rate that covers all the services identified in the SOO? 4.Describe other information about your facility and services to help demonstrate your capacity and capabilities, including the location of the facility, number of beds, etc. 5.Indicate your size status per the SBA guidelines (see http://www.sba.gov/contractingopportunities/officials/size/index.html). 6.Other recommendations and comments are welcome. 2.0 THIS IS A REQUEST FOR INFORMATION (RFI) ONLY. This RFI is issued solely for information and planning purposes - it does not constitute a Request for Proposal (RFP) or a promise to issue an RFP in the future. This request for information does not commit the Government to contract for long term TBI residential rehabilitative care services. Further, the VA is not seeking proposals and will not accept unsolicited proposals for these services. Responders are advised that the U.S. Government will not pay for any information or administrative costs incurred in response to this RFI; all costs associated with responding to this RFI will be solely at the interested party's expense. Not responding to this RFI does not preclude participation in any future RFP, if any is issued. If a solicitation is released, it will be synopsized on the www.fbo.gov. It is the responsibility of the potential offerors to monitor these sites for additional information pertaining to this requirement. 3.0 DUE DATE: Submittal of requested information is due not later than 3:00pm Mountain Standard Time, on Aug 18, 2014. We are trying to determine the best logistical locations for the best turn-out of interested parties in the range of dates of August 25, 2014. This will be held in Philadelphia, PA. Group meeting will be held in the morning and one on one will be held in the afternoon if needed. The site has not been confirmed. Please email Sheila Kiker, Contract Specialist, at sheila.kiker@va.gov for details. 5.0STATEMENT OF OBJECTIVES 5.1 Overall Objectives: The overall objective of the Assisted Living Pilot Program for Veterans with Traumatic Brain Injury services (AL-TBI) is to enroll eligible Veterans with moderate to severe TBI to promote retention and/or improvement in skills related to activities of daily living (ADL) or instrumental activities of daily living (IADL) and the social, behavioral and adaptive skills necessary to enable the Veteran to maintain functional gains and stability or potentially to reside in a non-institutional setting. This includes several levels of care: 5.1.1 Neurobehavioral Rehabilitation - designed with a primary focus on teaching behavioral alternatives to various unsafe or maladaptive behaviors. Neurobehavioral programs generally use an interdisciplinary treatment approach that may include use of behavior modification, medications, socialization skills training, substance abuse treatment and family therapy. An Individualized Plan of Care (IPOC) shall be developed and written for each Veteran to address specific goals related to medical, behavioral, cognitive, and emotional areas of concern. The goal of the program is to enhance participation in daily routine and community activities and to maintain stability and quality of life. 5.1.2 Neurobehavioral Rehabilitation - designed with a primary focus on teaching behavioral alternatives to various unsafe or maladaptive behaviors. Neurobehavioral programs generally use an interdisciplinary treatment approach that may include use of behavior modification, medications, socialization skills training, substance abuse treatment and family therapy. IPOCs shall be developed and written for each Veteran to address specific goals related to medical, behavioral, cognitive, and emotional areas of concern. The goal of the program is to enhance participation in daily routine and community activities and to maintain stability and quality of life. 5.2 The following range of rehabilitation services shall be available on-site or in the community as specified below. Services are to be provided 7 days a week as approved by VA and specified in the Veteran's IPOC. On-site rehabilitation services are those services that are located on the Contractor's physical site where the patient is residing. Community rehabilitation services are those services that the Contractor provides according to the patient's IPOC using a community provider. 5.2.1 The following rehabilitation services are required on site: (look at CARF standards for staffing) "Medical services and oversight for TBI "Registered nurse available 24 hours a day, 7 days a week "Social work/case management "Medication management and oversight "Social and recreational activities "24 hour security and staff availability "Cable, computer and internet access 5.2.2 The following rehabilitation services may be provided on site by Contractor providers, on site using community providers or in the community using community providers: "Physical Therapy "Speech Language Pathology "Occupational Therapy "Psychology/Neuropsychology oTherapeutic intervention to address mental health comorbidities of TBI including depression, anxiety disorder, adjustment, Post Traumatic Stress Disorder, other combat-related stress disorders and to assess cognitive functioning "Behavior Modification "Independent Living Skills training "Vocational counseling "Transportation with Attendant Services, for VA medical appointments, emergency care and recreational activities that are included in the Veteran's IPOC. "Health and wellness promotion and education "Gym and exercise facilities "Family support services 6.0 Contract Objectives: The objective of this procurement is to result in multiple contracts with providers of suitable residential rehabilitative care that meet the objectives of this SOO. Offerors must submit a Performance Work Statement in response to the solicitation that demonstrates how they will meet the objectives of this SOO. Contract objectives include, but are not limited to, the following: a.Multiple contracts are anticipated with multiple contractors in order to create a broad facility base for the patients. b.Contracts will be of an Indefinite Delivery/Indefinite Quantity type because identification of the patient population is an ongoing process of the AL-TBI contract. c.Specific orders for care will be placed by authorized coordinators at individual VA Medical Centers in coordination with the AL-TBI Program Manager. d.Selection of a contractor for receipt of an order will be at the discretion of the VA Medical Center placing the order, the AL-TBI Case Manager, the AL-TBI Program Manager and the COR. e.It is anticipated that the contracts will be awarded with a base year with four (1) year options. 7.0 Program Objectives: a.Contractors must furnish a care management system that will provide programmatic schedule and performance information to the Government throughout the AL-TBI performance. b.Establish a sound risk management system. Mitigate risk to the maximum extent possible, especially on software development efforts through integration of metrics to monitor program status. c.Establish a comprehensive communication management approach with the appropriate VA AL-TBI Case Manager and the AL-TBI Program Manager. 8.0 Specific Objectives: a.In addition to the levels of care outlined above the Contractor shall also provide the following services specified in the IPOC for each resident: i.Laundry and housekeeping services; ii.Meals and snacks plus assistance in eating, as needed; iii.Medication oversight, as needed; iv.Access to emergency services, as needed; and v.Other services or supplies commensurate with the VA authorized level of care, without extra charge. b.Contractor shall maintain certification or approved status as a provider in the Medicaid and Medicare programs, licensed or approved and in good standing with State licensing bodies, and full accreditation by a national accreditation body such as the Commission on Accreditation of Rehabilitation Facilities (CARF), c.The Quality Assurance Plan (QAP) will provide for routine measurement and analysis of clinical, fiscal and administrative program quality elements. Action plans are developed and implemented as necessary. The minimum quality elements that must be monitored include: i.Patient and/or family concerns ii.Annual verification that the Contractor remains in good standing with Federal and State licensing, certifying agencies, and national accrediting bodies. d.Upon award contractors must submit for VA approval their Safety and Sanitation practices that comply with the provisions of the National Fire Protective Association's NFPA 101, Life Safety Code (1994 edition) and the NFPA 101A Guide on Alternative Approaches to Life Safety (1995 edition). e.The VA, at its sole option, will monitor the professional care and fiscal and administrative management of services provided to VA beneficiaries under this contract, through one or any combination of the following methods; reviews of State agencies reports, site visits by a VA AL-TBI inspection team, telephone contact to ensure patients are receiving the services according to the Contractor's plan, and/or on-site monitoring of patient's care by the AL-TBI Case Manager. Patient and family satisfaction with these services will be assessed. It is agreed that the Contractor shall provide VA with copies of all State agency/other accrediting body reports, including reports of annual site surveys when requested. All medical records concerning the Veteran's care shall be readily accessible to VA without additional cost to VA. Upon discharge or death of the patient, medical records will be provided to the VA. f.It is agreed that the Contractor will collaborate and cooperate with VA medical center staff and the pilot program management staff in the conduct of the evaluation of the pilot program. g.The contractor shall provide the VA Contracting Officer's Representative (COR) annually or upon request any updates and any changes to the items listed below. The annual report is due fourteen (14) calendar days before the start of the period of performance for each option period. Other ad hoc reports that may be requested by the CO and/or COR shall be due within fourteen (14) calendar days of the request, unless otherwise specified. "Annual report "Updates to the contractor's QAP to include any enhancements, when applicable, to the program of services; "Emergency preparedness plan; "Facility Listing with Attestation Statement certifying that all facilities under the contract meet the following requirements: "Current applicable federal, state, and local licenses and certifications; "All Contractor personnel certification requirements; and "Verification of current CARF accreditation for all facilities under the contract; h.The Contractor shall report the following events via email to the assigned VA Case Manager and COR as soon as possible but within 24 hours of the occurrence, unless otherwise specified below. Email notifications shall be categorized as "High Priority" and include a designation in the subject line of "Action Required" or No Action Required," as applicable for VA action. "Reportable events as applicable by federal, state, and local requirements; "Veteran Safety event as defined in VHA Handbook 1050.1 at http://www.va.gov/vhapublications/ViewPublication.asp?pub_ID=2389; "Any significant change in the Veteran's physical or mental health condition; "Refusal of services by the Veteran or Veteran's caregiver; "Hospitalization - Veterans receiving care under this contract, who require acute hospital care, shall be admitted to an appropriate VA Medical Center (VAMC), as determined and authorized by VA. If, in an emergent situation, the Veteran must be admitted to a non-VA medical center, the Contractor shall notify the assigned VA Case Manager, the VA Business Officer at the affiliated local VA Medical Center, and the COR as soon as possible but within 24 hours of admission. "Patient self-discharge Against Medical Advice (AMA); "Death of Veteran; "Inability of the Contractor to deliver care or services as ordered; "Any inappropriate disclosure or release of Veteran information for any Veteran treated under this agreement that would constitute a violation of the Health Insurance Portability and Accountability Act of 1996 (HIPAA); "Formal notice of any action affecting the accreditation and certification of the facility by CARF or any other action affecting any of the facilities' federal or state licenses, certification, or accreditation; "Any change in a contracted facility's ownership or business address or any transfer of control of the facility's management or operations to any third party, within five (5) calendar days of knowledge; "Patient request to drive a personal vehicle or possess a personal vehicle on the Contractor site is prohibited and shall be reported to the COR. i.Once notified by the assigned VA Case Manager and/or COR that their facility has been selected for placement, the Contractor must evaluate the referred Veteran and create a recommended IPOC. This shall be submitted to the VA Case Manager and COR as soon as possible but no later than ten (10) business days after notification. Once the Veteran is admitted to the Contractor facility, the Contractor shall submit an updated IPOC and a schedule of the services to be provided weekly electronically via email every thirty (30) calendar days to the assigned VA Case Manager and COR. The Veteran's family and/or guardian should also be provided with or have access to the weekly schedule of services every thirty (30) calendar days. j.Referral for traumatic brain injury care services shall be initiated by the Veteran's VA health care team.
