Notice Type: Intent to Sole Source Intended Awardee: American Society of Addiction Medicine 4601 No. Park Ave., Ste. 101 Upper Arcade Chevy Chase, MD 20815 Title: Advancement of Standards of Care and Performance Measures for Addiction Medicine Solicitation#: 14-233-SOL-00607 Date: August 8, 2014 Notice of Intent: The Department of Health and Human Service (DHHS), Substance Abuse and Mental Health Services Administration (SAMHSA), Center for Substance Abuse Treatment (CSAT) intends to award a purchase to support SAMHSA's efforts to identify the goals of the Practice Improvement and Performance Measures Action Group (PIPMAG) and come to consensus on target outcomes of addiction treatment and identify how addiction specialist physicians can influence those outcomes through direct clinical care and through care system management. In Accordance with 10 USC 2304(c) (1), as implemented by FAR 13.106-1 (b)(1)(i) the American Society of Addiction Medication (ASAM) is the only responsible source able to satisfy agency requirements, it is likely that award to any other source would result in unacceptable delays. The proposed work builds on a follow-on contract for subsequent years. The Proposed Contractor has provided this service for years and has the experience, knowledge, materials, and past relationships to successfully complete this requirement. ASAM is the leading professional society representing addiction physicians and has held a seat in the American Medical Association House of Delegates since 1988 and has been a convener of consensus development meetings for many years. To have these standards and performance measures come from the leading professional society representing addiction medicine will assure that they are viewed as credible by the relevant stakeholders and will also assure adoption by the targeted physician audience. Furthermore, as the leading professional society for addiction physicians, ASAM can engage representatives of other related medical societies to ensure broad participation and adoption by organized medicine. ASAM has a long track record of working successfully with all of these associations and the other organizations necessary for the development of standards of care, implementation of standards of care and performance measures related to the standards. Background: Health Care Reform calls for quality improvement and performance measures (Title III Improving the Quality and Efficiency of Health Care, Part 2 National Strategy to Improve Health Care Strategy, Section 3013 Quality Measure development and Section 3014 Quality Measurement). SAMHSA/CSAT is committed to improving the quality of addiction care provided to clients with substance use disorders. Among the Clinical Quality Measures proposed for Medicare and Medicaid Eligible Professionals beginning with CY 2014, Initiation and Engagement of Alcohol and Other Drug Dependence Treatment is the only measure that would assess addiction physicians performance (with the assumption that Screening, Brief Intervention, and Referral for Treatment (SBIRT) for Alcohol Misuse and Medical Assistance with Smoking and Tobacco Use Cessation are primarily conducted by primary care providers). The Practice Improvement and Performance Measures Action Group (PIPMAG) was established by ASAM to address the need to define the standards of care for addiction physician specialists and to identify a preliminary set of associated performance measures. The focus of PIPMAG is on the clinical and administrative activities of physicians who are addiction specialists (addiction medicine physicians and addiction psychiatrists), in the care of persons with addiction or substance-related health conditions. Each of these efforts and initiatives, including those involving the patient placement criteria, included a wide range of stakeholder groups including the American Medical Association (AMA), American Psychiatric Association (APA), American Academy of Addiction Psychiatry (AAAP), American Osteopathic Academy of Addiction Medicine (AOAAM), etc. The successful development and adoption of standards of care and performance measures in other medical specialties has been achieved through the leadership of professional medical societies. The American College of Cardiology (ACC), for example, has been developing standards of care and performance measures for cardiovascular care for over 25 years. ACC's performance measures have been accepted by CMS for its pay for performance program. ACC has used its performance measures to launch national quality improvement programs such as the Door to Balloon program. The Door to Balloon initiative has engaged physicians and other providers across the country to reduce the amount of time for heart attack patients from the door of the ER to the cath lab for an angioplasty (i.e., balloon). It has been ACC's leadership that has engaged physicians and other providers in achieving significant improvements in patient care. Description: The first year has been devoted to developing standards of care as well as eliciting input from ASAM members and other stakeholders. The second year was devoted to identifying and describing proposed measures. The third year will involve specifying a subset of the measures identified in year 2 and pilot testing them using administrative data. Dissemination of the standards occurred in year 2 along with dissemination of the measure descriptions. At the conclusion of year 3, the specified measures will be disseminated to payers and other groups for pilot implementation. ASAM will: (1) identify the goals of the Practice Improvement and Performance Measures Action Group (PIPMAG) and elicit input from ASAM members and other stakeholders; (2) develop standards of care and associated performance measures; (3) validate the performance measures; and (4) disseminate those standards of care and performance measures to its members and non-member stakeholders over a two-year period.   Contractors possessing equal capabilities are encouraged to respond in writing no later than August 20th, 2014. All responses should provide at a minimum: 1.
