- PAST PERFOEMANCE QUESTIONNAIRE TERMS AND CONDITIONS • This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulations (FAR) Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. • Solicitation A14093 is hereby issued as a Request for Proposal (RFP) and incorporates provisions and clauses in effect through Federal Acquisition Circular 2005-76, effective 25 July 2014. • Set-Aside and NAICS Code: This solicitation is being issued as a 100% Total Small Business Set-Aside under NAICS Code 492210 with a small business size standard of $27.5 Million. • Description of Requirement and items: The Internal Revenue Service's Program Support Branch intends to issue one or more Blanket Purchase Agreements (BPA) to support the Treasury Departmental Offices' need for Newspaper & Magazine Delivery Services. Calls issued against the BPA(s) will be Firm Fixed Price. In addition to the required information required under FAR 52.212-1, offerors shall also provide the following information in their response:  Company Name and Address  Company POC & Title, Telephone No., and Email Address  DUNS No. All work performed under the BPA will be performed through the issuance of Call Orders. Each Call will be individually funded in accordance with the BPA terms and conditions. • Ordering Period: Calls may be placed against the BPA throughout the entire performance period of the BPA. The Period of Performance shall be from date of award of the BPA to 31 August 2019. There are no option periods associated with the BPA and either party may discontinue the agreement at any time upon 30 days written notice by either party. • Delivery Dates & Place of Delivery: The contractor shall deliver a specified number of print newspapers to the designated locations each weekday morning by 7:00 am for "Business Locations" and 5:00 am for "Residential Locations," excluding federal holidays. Delivery of newspapers published on the weekend will occur on Monday, or the next business day if Monday is a federal holiday.
