Description
FAR 52.212-5 Attachment 4 FAR 52.212-4 Attachment 3 FAR 212-3 Attachment 2 CLIN Structure Page Synopsis: 1. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. 2. Solicitation F2Q3144171A901 is being issued as a Request for Quotation. 3. This solicitation incorporates provisions and clauses for commercial items in effect through Federal Acquisition Circular 2005-36. 4. This acquisition is reserved for a Small Business concerns; however, in the event that no Small Business concerns can meet the requirement the set-aside will be withdrawn and an award will be made to a Large Business. The associated NAICS code for this acquisition is 611710, standard industrial classification 611, small business size $15 Million. 5. This is a firm fixed price requirements type contract for a Marriage Retreat Presenter to provide and present marriage curriculum which addresses marital health, wellness, communication, and resiliency in the military context. Topics for consideration may include, but are not limited to, deployment preparation, separation, and reunion after the deployment. These services will be required during the Marriage Resiliency Retreat being held from 12-14 September 2014. The alternate date for the event is 19-21 September 2014. CLIN 0001: Resiliency Marriage Retreat Presenter Requirements : The Government reserves to right have coordinating chaplains select a presenter as provided for in the requirements called out below, or present their own curriculum. Requirements for optional presenters are: a. Presenters shall be individuals or groups and must be experienced in presenting programs for both marriage retreats and singles retreats in a military environment. Presenters with military specific ministries are desirable. b. Presenters shall provide and present their curriculum. This means that the presenter provides all media for the most effective presentation of their product. Media includes, but is not limited to, multimedia presentations, books, pamphlets, DVDs, or other items which attendees may carry when they depart the retreat. c. Curriculum shall be spiritually based for appropriate use in this chaplain sponsored program. d. Marriage curriculum shall address marital health, wellness, communication, and resiliency in the military context. Topics for consideration may include, but are not limited to, deployment preparation, separation, and reunion after the deployment. e. Curriculum shall be approved by the JBLE Chaplain or Deputy JBLE Chaplain. Approval may require interview and/or brief presentation. f. The presenter may design the marriage retreat programs using the examples below or design their own. Presenters shall be competent and possess the necessary skills to conduct the retreats in a manner that is sensitive to the various spiritual philosophies which exist among military personnel without compromising the presenter's curricula. h. Presenters shall be competent and possess the necessary skills to conduct the retreats in a manner appropriate to the post-DOMA (Defense of Marriage Act) environment of the Department of Defense. i. Presenters shall conduct retreats with dignity and respect for the service members and families in attendance. 6. Offerors quotes must be submitted with pricing according to individual CLIN structure in the attached page (Attachment 1). Offerors must submit a resume attesting to counseling and/or previous pastoral experiences. The Government reserves the right to conduct interviews with responsive offerors prior to making an award. 7. The following provisions in their latest editions apply to this solicitation: FAR 52.212-1, Instructions to Offerors, Commercial. FAR 52.212-2, Evaluation - Commercial Items. The following factors will be used to evaluate offers: (1) Credentials in Resume (2) Curriculum (3) Price 8. Government intends to award a contract without discussions with respective offerors. The Government, however, reserves the right to conduct discussions if deemed in the best interest of the Government. 9. Offerors are to include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications - Commercial Items with their offer. A copy has been posted at www.fbo.gov. (Attachment 2). 10. The following clauses in their latest editions apply to this solicitation: FAR 52.212-4,Contract Terms and Conditions -- Commercial Items. Contract Terms and Conditions -- Commercial Items (Feb 2012) (Attachment 3). FAR Clause 52.212-5 CONTRACT TERMS AND CONDITIONS REQUIRED TO IMPLEMENT STATUTES OR EXECUTIVE ORDERS- COMMERCIAL ITEMS (JAN 2014) (Attachment 4). DFARS 252.203-7000, Requirements Relating to Compensation of Former DoD Officials. 252.203-7002, Requirement to Inform Employees of Whistleblower Rights 252.212-7001 (Deviation), Contract Terms and Conditions Required to Implement Statutes or Executive Order 252.232-7003, Electronic Submission of Payment Requests 252.232-7010, Levies on Contract Payments Additional FAR clauses cited within the above clauses, which are applicable, will be incorporated into the solicitation and the resulting contract. 11. The Department of Labor Service Contract Act Wage Determination WD 05-2544 (Rev.-15) dated 06/25/2013 (for the Hampton Roads Area) is currently incorporated into this solicitation; however, the current prevailing Wage Determination for Illustrator III/Presenter will be incorporated at the time of award. 12. The following additional provisions are incorporated by reference: 52.219-1 Alt 1, Small Business Program Representations 13. All information relating to this solicitation, including changes/amendments, questions and answers will be posted on the following website: (www.fbo.gov). Formal communications (such as request for clarifications, questions, and/or information) concerning this solicitation must be submitted in writing or via e-mail to: lucinda.williams.1.ctr@us.af.mil. 14. Offerors are requested to submit questions to the E-mail address noted below, not later than 11:00 P.M. EST, 08 August 2014. Terms of the solicitation and specification remain unchanged unless the solicitation is amended in writing. If an amendment is issued, normal procedures relating to the acknowledgement and receipt of the solicitation shall apply. Award can only be made to contractors registered in System for Award Management (SAM) at: https://www.sam.gov. Awarded vendor must invoice through Wide Area Work Flow (WAWF), an online invoicing system https: //wawf.eb.mil/ after completion of event.
