Description

This is a pre-solicitation notice for a contractor to provide all personnel, equipment, supplies, facilities, transportation, tools, materials, supervision, and other items and non-personal services necessary to perform Histology Block and Stain Reference Lab Service for the Phoenix VA Health Care System (PVAHCS). This requirement comes as a result of a new requirement at the Phoenix VA Health Care System (PVAHCS) Laboratory Department to purchase services from qualified reference labs in the histotechnoloyg preparation of surgically-obtained tissue samples for pathological review. The Contractor must continuously hold a Certificate of Compliance or Certificate of Accreditation from the Centers for Medicare & Medicaid Services as meeting the requirements of the Clinical Laboratory Improvement Amendments of 1988 or must demonstrate accreditation by a regulatory agency(s) with deemed status from the Centers for Medicare & Medicaid Services, e.g. The College of American Pathologists, and/or other state regulatory agencies, as appropriate, and as mandated by federal and state statutes. The Contractor must maintain valid certifications during the entire performance period of this contract. The reference lab should embed, stain, cut and coverslip the specimens; and then return the specimen to the VA Health Care System in a timely manner for pathologist analysis. The solicitation will be posted to http://www.fedbizopps.gov/ on or about August 15, 2014. This acquisition is 100% set-aside for small business. The applicable NAICS code for this acquisition is 621511 and the size standard is $32.5 million. The proposed contract action shall cover a base period of performance from October 1, 2014 to September 30, 2015 with four one year options. The government anticipates award of a firm-fixed price requirements contract as a result of the solicitation. The solicitation may be downloaded from FedBizOpps, since copies of the solicitation will not be available for mailing. All prospective offerors must be registered in the System for Acquisition Management (SAM). Registration may be accomplished at http://www.sam.gov/.
