- Wage Determination Past Performance Questionnaire 52.212-3 provision Statement of Work SBAHQ-14-Q-0054 Please see attachments This is a combined synopsis/solicitation for commercial items prepared in accordance with FAR Subpart 13.1 and the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number for this requirement is SBAHQ-14-Q-0054 and is being issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-76. This will be a fixed price contract; this is a total Small Business set-aside; NAICS code is 541618 with a small business size standard of $15.0. Contractors must be registered and active in the System for Award Management (SAM) database (https://www.sam.gov) at time award is issued and throughout performance unless an `applicable exemption exists. SBA's American Supplier Initiative (ASI) is a government-wide effort to grow small businesses, create jobs and to ensure that America has a strong, deep and diverse supply chain. The American Supplier Initiative is a call-to-action to the private sector to invest in their supply chains through small businesses. The initiative aims to address three key areas in which small business need help in order to become successful suppliers in the private sector: access to mentorship and counseling services, increased market and revenue opportunities, and ready sources of capital to fund their growth. As a part of the ASI, the SBA is holding a series of supply chain events, in collaboration with federal agencies and private companies, throughout the country to provide the tools and resources needed for small businesses to become effective suppliers and connect small businesses with federal and commercial supply chain opportunities and banks through matchmaking. Small businesses who are suppliers to large corporations reported revenue growth of more than 250 percent and employment increased by more than 150 percent on average.
