Description

PLEASE READ THIS ENTIRE NOTICE CAREFULLY AS IT CONSTITUTES THE ONLY NOTICE THAT WILL BE ISSUED. This is a combined synopsis/solicitation for commercial items prepared under FAR Part 13 Simplified Acquisition Procedures, and in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Registration at http://www.SAM.gov (SAM) is required in order to be eligible for award of Government contracts. Paper copies of this solicitation are not available. Please see all attachments. Solicitation Number: VA248-14-Q-1720, the solicitation is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through the most current Federal Acquisition Circular. This RFQ is issued as a 100% Service-Disabled Veteran-Owned Small Business (SDVOSB) set-aside, associated NAICS code 339112, small business size standard 500 employees. SDVOSB must be registered on vetbiz.gov. The Department of Veterans Affairs, Network Contracting Office (NCO) 8, on behalf of the James A. Haley VA Medical Hospital, Tampa, FL, has a need for a VISN 8 Clinical Information System (CIS) Expansion (Tampa PCU) Medical Grade 19" Touchscreen All-In-One PCs and associated installation, training, and warranty. Specific requirements are listed on the attached Schedule of Supplies and Services, brand name or equal, and salient characteristics on the Statement of Work (hereafter referenced as SOW). Provision at 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition with addenda. Provision at 52.212-2, Evaluation-Commercial Items applies to this RFQ, evaluation criteria is LPTA, lowest price technically acceptable. Please include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items, with the quote if not already registered in the System For Award Management (SAM) at www.sam.gov. Clause at 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition with addenda. Clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items, applies to this acquisition with additional FAR and VAAR clauses. Quotes are due 18 August 2014 @ 4:00pm EST via email to linda.baldwin1@va.gov. Questions will not be accepted after 14 August 2014 @ 3pm EST.
