Description

STATEMENT OF WORK (SOW) Northport VAMC 79 Middleville Road Northport, NY 11768 A.GENERAL INFORMATION 1. Title of Project: Oxygen carts, tanks and regulators 2. Scope of Work: The facility emergency management department with the coordination of warehouse operations, is seeking ancillary oxygen equipment in the event an emergency arises and we are unable to get deliveries of oxygen to our facility. We must have the ability to store excess carts, oxygen cylinders and regulators to sustain operations. 3. Background: Due to the frequent storms in the northeast over the past few years, where power shortages and truck delivery delays have been seen, it's imperative that the hospital have adequate stock of oxygen equipment to distribute throughout the hospital. Additional oxygen equipment will facilitate Emergency Management preparation for future pandemic or emergency situations. 4. Performance Period: [Firm-Fixed-Price] The contractor shall complete the work required under this SOW in _60__ calendar days or less from date of award, unless otherwise directed by the Contracting Officer (CO). If the contractor proposes an earlier completion date, and the Government accepts the contractor's proposal, the contractor's proposed completion date shall prevail. Delivery to the Government site shall not take place on Federal holidays or weekends unless directed by the CO. 5. Type of Contract: Firm-Fixed-Price. Contractor provides all resources necessary to accomplish the deliverables. Specifications "Seeking 2 "E " Size cylinder carts, capable of holding 50 cylinders. It needs to be made of steel and comes with 4 6" casters for the purpose of having mobility options. "Seeking 2 "E or D" size cylinder carts, capable of holding 24 cylinders with a sound deadening kit. It needs to be made of steel and comes with 4 6" casters for the purpose of having mobility options. "Seeking 50 "E" size aluminum oxygen cylinders "Require 200 seals - Yoke, brass Viton "Seeking 50 oxygen regulators,.5-15lpm, DISS connectors, BCGA870 rated with a ErgoKnob feature. *NOTE: All Cylinders Must Be Aluminum Contracting Office Address: VISN 3 Network Acquisition & Logistics James J.
