- Request For Quote Document Women's Health Nurse Provider Services (The quote shall be valid through 31 October 2014) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a separate Request for Quote (RFQ) is attached. The solicitation number is FA3047-15-T-0003 and is issued as a Request for Quote (RFQ). This solicitation document and incorporated provisions are those in effect though Federal Acquisition Circular 2005-76 and DFARS change notice 20140721. This requirement will be a 100% Woman Owned Small Business (WOSB) set aside. This NAICS code is 561320 and the size standard is $27.5M. This is a requirement for Women's Health Nurse Provider Services at San Antonio Texas Department of Defense (DoD) Military Treatment Facilities (MTF). Special Notes and Instructions: The vendor shall submit the response to this RFQ via email to: SrA Rachael Avery, E-mail: rachael.avery.2@us.af.mil; Ph: (210) 671-4882, no later than 4:00 P.M CST on Sep 15, 2014 - Monday in order to be considered timely. Any quote, modification, revision, or withdrawal of a quote received after the exact time specified for receipt of quote is "late" and will not be considered, unless the Contracting Officer determines that accepting the late quote would not unduly delay the award and acquisition or it was the only quote received. 1. Basis for Award. 502d CONS will issue a purchase order to the responsible Quoter whose quote conforming to the solicitation will be most advantageous to the Government for performance of all line items listed in the RFQ. 2. Price Evaluation: The Government will conduct a price evaluation of all quotes in accordance with Addenda to 52.212-2 Evaluation - Commercial Items of the attached SF 1449 - Solicitation (Attachment 1). 3. Mandatory Registrations. To be eligible for award, registration with the System for Award Management (SAM) must be current (legacy for On-line Registration and Certifications Application (ORCA) and Central Contractor Registration (CCR) registrations). To register or update former CCR and Reps & Certs that are not current go to https://www.sam.gov/portal/public/SAM/ and provide mandatory information. Attachment: 1. SF 1449 - Solicitation
