Description

DOL WAGE DETERMINATION APPENDIX A PERFORMANCE WORK STATEMENT (PWS) REQUEST FOR QUOTE This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with the additional information included in this notice. This announcement constitutes the only solicitation; proposals are being required and a written solicitation will not be used. Non-Personal Services: The Air Force intends to solicit and issue an award for the following services: The contractor shall provide all labor, tools, equipment, test equipment, material, parts, quality control, transportation, personal protective equipment, clothing including reflective clothing (e.g.belt, vest), hearing and eye protection, including prescription safety glasses, safety harnesses, fall protection, lifting equipment and any other incidentals necessary to lease, deliver, install and provide maintenance support services on De-Ionized Water Regeneration Treatment Tanks in Bldg. 3001, at the Oklahoma City Air Logistics Complex (OC-ALC). The contractor shall provide and install a set of one each De-ionized Water Regeneration Treatment Tanks consisting of a Cation, an Anion and a Carbon Tank. The contractor shall be responsible for insuring all services and equipment provided under this contract are IAW the terms and conditions outlined herein and conform to the attached Performance Work Statement (PWS). A Firm Fixed Price (FFP) contract with a Cost Line Item for Change Out Services is contemplated. The proposed Period of Performance is 22 September 2014 through 21 September 2015. Estimated award date is 19 September 2014. Award will be conducted and evaluated under the provisions of FAR Part 12, Commercial Items, and FAR 13.5, Simplified Acquisition Procedures. The responsible offeror must be registered in the Online Representations and Certifications ‘System Award Management' (SAM) database (available at: https://www.sam.gov) system per FAR 52.212-3, and submit a completed copy of the Offeror Representations and Certifications - Commercial Items with their quote per FAR 52.212-1.
