Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested. The solicitation number is: VA741-14-Q-0223, this solicitation is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-69. This requirement is Set Aside exclusively to Small Businesses; only qualified Offerors may submit bids. The associated North American Industrial Classification System (NAICS) code for this procurement is 332510 with a 500 Employee size standard. The Center of Excellence in Suicide Prevention is in need of the following items: Contract line item number 0001: 2.2.1 Furnish 1,162,500 cable gun safety locks, instructions for use, and educational materials; Items must be ASTM compliant, Padlock body types will be steel or steel over plastic, positive-cam locking mechanisms, or similar, two keys provided with each lock. 2.3.1The contractor shall provide and maintain a detailed project management plan/schedule (PMP). The contractor will ensure that the work breakdown elements incorporate a sufficient level of detail to adequately delineate a comprehensive overview of items to be delivered 2.3.2The contractor shall provide a quarterly statement, or as requested by COR, which shows quantity of safety gun locks and materials shipped and the cost of shipping those items DELIVERY SPECIFICS: 2.2.2Shipping/distribution to sites nationwide as noted in attachment A Gun Lock Distribution List Tri-annual Deliveries Anticipated Dates of Delivery to each Site: 11/2014 05/2015 09/2015 PCA East Contracting is seeking the services of a contractor for the purchase and delivery of Cable Style Gun Locks in Support of Suicide Prevention for the Veteran community. The VISN 2 Center of Excellence for Suicide Prevention (Center/CoE) at the Canandaigua VA Medical Center is looking to obtain a contract for the purchase of 1,162,500 cable gun safety locks, to include instructions for use, adult and family oriented gun safety education materials, vendor shipping and distribution of gun locks and materials to multiple sites nationally. The purchase and distribution of gun safety locks has been an ongoing effort for the Center in collaboration with the Suicide Prevention Office at VA Central Office, since 2009. Period of Performance: September 15, 2014 to September 14, 2015 Delivery of Items will be required for multiple facilities nationwide: See Attached Gun Lock Distribution List for delivery quantities and specifics. The selected Offeror must comply with the following commercial item terms and conditions, which are incorporated herein by reference: FAR 52.212-1, Instructions to Offerors - Commercial Items, applies to this acquisition; FAR 52.212-2 Evaluation-Commercial Items applies to this acquisition; FAR 52.212-3, Offeror Representations and Certifications - Commercial Items - the selected Offeror must submit a completed copy of the listed representations and certifications; FAR 52.212-4, Contract Terms and Conditions - Commercial Items; FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items, paragraph (a) and the following clauses in paragraph (b): 52.204-10, 52.219-6, 52.219-28, 52.222-3, 52.222-21, 52.222-26, 52.232-35, 52.222-36, 52.222-37, 52.223-18, 52.225-13, 52.232-34, 52.222-41, 52.222-44.
