Description
Solicitation 246-2014-P-0021 This contains Instructions to Offerors, Terms and Conditions, Bid Schedule, Statement of Objectives Standard Form SF 18 (must be completed and signed and accompany proposal) Contractor shall provide a firm fixed price proposal that includes a Scope of Work that details all labor, materials, supplies, tools, equipment, transportation, supervision, approach and all other necessary items and services required for replacing the existing Dietary Ventilation Hood with a NFPA compliant ventilation hood in the kitchen of the Lawton Indian Hospital. The Scope of Work shall provide a clear understanding and approach, meeting the requirements, objectives and constraints per the Scope of Objective. This Solicitation provides a Scope of Objective only. It is the responsibility of any interested Offeror to provide a Scope of Work with their proposal. Failure to include in your Scope of Work to address each Objective and Constraints contained in the Scope of Objective shall be reason for the Contracting Officer to reject proposal as non-responsive. This procurement is offered as a Small Business set-aside. A Small Business set-aside is defined in FAR Part 19.301-1, Representation by the offeror. To be eligible for award as a small business, an offeror must represent in good faith that it is a small business at the time of its written representation. An offeror may represent that it is a small business concern in connection with a specific solicitation if it meets the definition of a small business concern applicable to the solicitation and has not been determined by the Small Business Administration (SBA) to be other than a small business. All work is to be performed at the Lawton Indian Hospital, Lawton, Oklahoma The dollar magnitude is estimated between $100k -$200K NAICS 238220 applies and size standard is $15M The Performance Period is estimated not to exeed 90 calendar days after receipt of the Notice to Proceed. Any questions or clarifications generated from the Site Visit shall be submitted no later than 9:00 am, Wednesday, August 27, 2014.
