Small Business Set-Aside: Total Small Business
Description: This is a combined synopsis/solicitation for commercial items and services is prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation is being issued using Simplified Acquisition Procedures. The solicitation number is VA119-14-Q-0237 and is issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-76 (Effective 25 July 2014). This is a Total Small Business set-aside. The associated North American Industrial Classification System (NAICS) code for this procurement is 511210 with a small business size standard of $38,500,000.00. Any Service Disabled Veteran Owned Businesses (SDVOSBs) or Veteran Owned Small Businesses (VOSBs) that responds to this requirement must be registered and verified in the VetBiz VIP database, http:www.vetbiz.gov, at the time of quotation submission in order to be considered award. This award is for a Firm Fixed-Price (FFP) Purchase Order (PO) for one (1) 12- month Base year period from the date of award, with two (2) twelve 12-month option periods. Each Contract Line Item Number (CLIN) will be priced to meet the need for all 143 medical centers throughout the VA enterprise. The list of contract line item number(s) (CLINs) and items, quantities and units of measure, including options can be found in Attachment 1- CONTRACT LINE ITEMS (CLINs). Description of requirements for the items to be acquired: The Department of Veterans Affairs (VA), Veterans Health Administration (VHA), Office of Quality, Safety & Value Credentialing & Privileging (OQSV) has identified a need for an internet enabled Clinical Privileging Library. The purpose of the Clinical Privileging Library is to provide guidance and grant specified medical and patient care services, so that the provider may practice within the scope of the individual's license and clinical competence. Clinical Privileging also provides medical staff and leadership the tool to comply with policy set forth by the Department of Veterans Affairs (VA) regulation, statute, and the Joint Commission. See Attachment 2- Product Description- Clinical Privileging Library for details regarding this requirement. Date(s) and place of delivery and acceptance FOB point: Delivery shall be 15 days after award. The period of performance shall be one (1) 12-month Base Year period from the date of award, with two (2) twelve (12) month option periods. Acceptance shall be by written approval and acceptance by the Contracting Officer's Representative (COR). FOB Point: Destination. The provision at 52.212-1, Instructions to Offerors - Commercial Items, applies to this acquisition and the addendum to the provision is attached. See Attachment 3- 52.212-1 Addenda- Instructions to Offerors - Commercial items. The Contracting Officer (CO) will determine best value and the basis on which award will be made is lowest price. The lowest priced quote will be evaluated to ensure that it meets the Minimum Technical Requirements (MTRs)-Attachment 2- Product Description- Clinical Privileging Library for details regarding this requirement. Include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with its offer. An offeror shall complete only paragraphs (b) of this provision if the offeror has completed the annual representations and certificates electronically via http://www.acquisition.gov. If an offeror has not completed the annual representations and certifications electronically at the System for Award Management (SAM) website, the offeror shall complete only paragraphs (c) through (o) of this provision.
