Description
The National Cemetery Administration is soliciting offers under Solicitation VA786-14-R-0235 for Black Hills National Cemetery, Sturgis South Dakota and FT. Bliss National Cemetery, El Paso Texas: 1.Submittal by Potential Offerors: Potential offerors shall submit at contractor expense, one (1) sample as outlined below. The sample shall be representative of the product to be used in the production of headstones and markers under the contract and shall conform to government specifications. Mark each sample on the back with permanent marker to show: contractor's name and solicitation number, email address and cemetery name. Any sample which arrives damaged will not be accepted. a.The sample shall be manufactured, inscribed, and packaged to prevent any damage during shipment. b.Sample shall be accompanied by a statement identifying the brand name and grit rating for the pad or stone used to produce the final finish. c.Failure to meet any of the requirements shall result in the rejection of the sample. d.The sample will establish the minimum acceptable product for all inscriptions under this contract and shall be the basis for comparison of all inscription services provided under this contract. Once submitted, sample become the property of the Department of Veterans Affairs and will not be returned. Submit sample and test results to the following address: Department of Veteran Affairs Memorial Programs Service Program Support Unit, Rm. 650 1575 Eye Street, NW Washington, DC 20005 e.All potential offerors who have submitted and received approval notification for a sample of the same type within the last 12 months may submit a copy of that approved sample review with the proposal bid to avoid submitting an additional sample. f.Sample shall be received by Memorial Programs Service (MPS) prior to closing date. If one or more of the sample are disapproved by MPS, a notification attempt will be made via phone and/or email to ensure you provide current information. Potential offerors shall have until closing date of the solicitation to resubmit another sample. The Government will not accept sample after the closing date. 2.Sample Required: a.Inscribed Upright Marble Headstone (U1) Sample: One (1) marble sample 24" x 13" x ¾" The following inscription shall be used for this sample: Emblem #25 (refer to drawings for actual emblem to be inscribed) MARSHALL P GRIFFIN Jr (refer to drawings for correct size of "r") 1st LT (refer to drawings for correct size of "st") US ARMY KOREA VIETNAM MAR 1 1932 OCT 4 2008 PURPLE HEART AIR MEDAL 101 ABN DIV b.Full size copies of drawings are available upon request at no additional charge to potential vendors. The request shall be e-mailed to the NCA Memorial Programs Service (MPS) Program Support Unit at MPSProgSupport@va.gov and shall include the contractor's name, mailing address, phone number, e-mail address, and the name / drawing number of the drawing(s) being requested. If hard copies of the drawings are desired, e-mail the request to MPS with sufficient time for the drawings to be mailed. MPS recommends vendors use full size drawings for determining work requirements. An extension to the solicitation's closing date WILL NOT be granted for failure to request the technical drawings with sufficient lead time. 1.The contractor shall be responsible for all work at the cemetery. This procurement is for Lowest Price Technically Acceptable. Evaluation criteria for this award will be in the following order: 1) Price, 2) Technical Qualifications, and 3) Past Performance. This acquisition is 100% set aside for Service Disabled Veteran Owned Small Business (SDVOSB). The NAICS Code is 327991. Size standard is 500 employees. Electronic solicitations will be available for download from http://www.fbo.gov on or about July 21, 2014. Responses to the solicitation are due on or about 2:00 P.M. on August 04, 2014. Any amendments to the solicitation will also be available at www.fbo.gov. Offerors are advised they are responsible for obtaining any amendments.
