- Reference Request for Proposal (RFP) Number: VA786-14-R-0324. The Department of Veterans Affairs, National Cemetery Administration is soliciting proposals from qualified and eligible Service Disabled Veteran Owned Small Business Concerns capable of providing all plant, labor, tools, parts, equipment, transportation, taxes, supervision, materials, etc ¦ to install and replace headstones at Fort Logan National Cemetery located at 4400 W. Kenyon Avenue, Denver CO 80236. The scope of work includes but is not limited to having a single contractor install and replace headstones in a manner that will present a clean, neat, and professional appearance throughout the Cemetery and meet or exceed minimum requirements of the Performance Work Statement. Services include transportation of removed headstones to an approved location on site, preservation of existing cemetery grounds, restoration of disturbed areas and performing day-to-day installation and replacement of headstones. The contract period of performance is for one base year consisting of twelve (12) months with four (4) individual one year option periods. The Government anticipates that contract performance will commence on/about October 1, 2014 and conclude September 30, 2015 with options being exercised on the anniversary date of September 1st. The total contract performance period will not exceed a total of five years from date of contract award. The point of contact and Contracting Officer for this procurement is Mr. Derek C. This is a best value type RFP issued in accordance with FAR Part 15 and the Government reserves the right to make contract award to other than the lowest priced offeror. The North American Industrial Classification Code for this acquisition is 561730 with a small business size standard of $13.0M. Currently, funds are unavailable for this acquisition, however, the Government anticipates funds will become available on/about October 1, 2014; nevertheless this project is subject to the availability of funds for next fiscal year. The Government anticipates issuing a commercial type RFP on/about August 29, 2014 with proposals being received on/about September 15, 2014. Interested parties are encouraged to visit www.fbo.gov to obtain copies of the solicitation documents as no hard copies of the solicitation documents will be furnished. In addition, prospective offerors are encouraged to visit the jobsite to familiarize themselves with existing conditions prior to submitting a proposal or offer in response to the RFP. Only one organized site visit will be conducted and information regarding the one and only organized site visit will be listed with the RFP. Prospective offerors are encouraged to submit questions to the Contracting Officer prior to RFP closing, and if deemed in the best interest of the Government, an amendment may be issued to answer any questions. Solicitation Number VA786-14-R-0324 has been assigned to this RFP and all correspondence regarding subject RFP should reference the solicitation number. Electronic or facsimile proposals will not be accepted and therefore it is incumbent on the offeror to ensure proposals are submitted timely and in accordance with solicitation clause 52.212-1 entitled 'Instructions to Offerors-Commercial Items.' Failure to submit proposals in accordance with submission instructions may exclude your firm from consideration of possible contract award. In order to be eligible to receive an award offerors must be registered at www.vetbiz.gov and must have an active account at www.sam.gov.
