This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice.The Solicitation number is 608-14-3-7077-0041 and is issued as a Request for Quotation (RFQ) unless otherwise indicated herein. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-76. The associated North American Industrial Classification System (NAICS) code for this procurement is 334419 with a small business size standard of 500 employees. This requirement is unrestricted and only qualified offerors may submit bids. All interested quoters may submit their quote to Mary.Bosch@va.gov. SALIENT CHARACTERISTICS Embletta Home Sleep Study System (A) Functions to be performed: The VAMC Manchester Sleep Study Program requires an Embletta Home Sleep Study System for ambulatory sleep study patients. VISN EDM has required having Manchester begin providing home sleep studies for Manchester and WRJ. This equipment interfaces with our current system. This function is currently being performed through outside providers feed out by the VA. The addition of this system to the Manchester VAMC will provide a higher level of service to our Veterans who are unable to test within our facility. (B) Performance required; or Supplier shall be able to respond to request for an Embletta Home Sleep Study System. All deliverables shall be sent to VAMC Manchester's Sleep Study service. Vendor will comply with FDA (Federal Drug Agency), OSHA (Occupational Safety Health Administration) and NFPA (National Fire Protection Association) regulations and guidelines to provide equipment that complies with safety standards for workplace use. Patient identifiable information (PII) shall not be shared with supplier. No new construction, IT lines, FTE, or consumable supplies required. (C) Essential physical characteristics: An Embletta Home Sleep Study System has been a proven source in the New England Healthcare system. The acquisition of this system will allow ambulatory sleep studies to be comfortable and reliable for our veterans. Technical Specs: Sampling Rate (Hz), 8000 Storage Rate (Hz), up to 500 Resolution (bit), 24 Recorded Channels, 12; Pressure, Sound,Gravity (X/Y/Z), bipolar ExG, Thermistor, Thoracic Effort (RIP), Abdominal Effort (RIP), DC (0 - 1 V), Battery, Event Button Recorded Channels Pulsoximeter, 7; SpO2, SpO2 B-B, SpO2 Quality, Pulse Rate, Plethysmogram, RD Quality, PPG Derived Signals, 7; Nasal Flow, Mask Pressure, Snore, Sound Amplitude, Body Position, Activity, Elevation (multiple additional traces derived by software) Internal Memory, 2GB Power Supply Properties, 2 x AA batteries, standard alkaline or rechargeable Recording Time, 24 hours Size (length x width x height), 107mm x 79mm x 19mm Weight, 153g (including batteries) Typical Operating Temperature, 16 °C to 37 °C The VHA NCO 01 - 00241 - SAO East requires the following items, Brand Name or Equal, to the following: LI01 - Part Number 012438 Embletta MPR PG SYS - XR-US-5pack quantity 4 LI02 - Part Number 2200102 High Speed Computer System Quantity 1 LI03 - Part Number 3022020 Sleep System Web Training Quantity 1 Solicitation and Buy Attachments For this solicitation, VHA NCO1 WRJ VAMC intends to conduct an online competitive RFQ. An offeror may submit information by email to Mary.Bosch@va.gov. New Equipment ONLY; NO remanufactured or "gray market" items. All items must be covered by the manufacturer's warranty. This solicitation requires registration with the System for Award Management (SAM) prior to award, pursuant to applicable regulations and guidelines.
