- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number W9114F-14-T-0094 is issued as a request for quotation (RFQ). Solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-50, effective 16 March 2011. Not a set-aside. North American Industry Classification System (NAICS) code for this solicitation is 423450. All equipment components must possess FDA Certification. Subfactor 2. Ability to provide types of preventative maintenance procedures, user training and warranty that shows the frequency and time committed to perform each type of task per shift, per day, per week, per month, per year to sustain an operational system. Subfactor 3. System must be able to interface with the MUSE V.8 System. Device must be compatible with the MUSE database in order to provide expedited workflow, analysis, access, and storage of patient data. Subfactor 4. System must be compatible with existing GE SEER Lite Extend Ambulatory ECG recorders. Subfactor 5. System server software must be Windows Server 2008 and be able to be loaded onto the LRMC virtual server. Subfactor 6. System application client software must be Windows 7 and be able to safely load to hospital based PCs. Subfactor 7. System must have web based ability to transfer recorded ambulatory ECG data from outlying clinics. Subfactor 8. System must be installed by vendor with assistance from Landstuhl Medical Center - System Administrators and Technical Support. Subfactor 9. Additional Ambulatory ECG Recorders must be compatible with existing recorders with the capability of 48 hour ECG recording, AAA battery powered, small, light and compact with accessories such as carrying case and leadwires compatible with existing inventory. Subfactor 10. The analysis system must possess: EK-Pro Analysis, Atrial Fibrillation Detection, QT Interval Measurements, ST Segment Measurements, Heart Rate Variability and Pacemaker Analysis. Subfactor 11. The analysis system must be able to compile reports to include histograms, trends, summary and actual ECG recordings for proper interpretation by the interpreting physician. Factor II - Price. The Government will select the quote that is based upon the lowest evaluated price, technically acceptable quote. All CLINs shall have been priced by the vendor. If the vendor failed to price all CLINs, the quote will not be considered for award. ADDENDUM TO FAR 52.212-2 The Government will award a Purchase Order resulting from this solicitation to the responsible vendor whose quote conforming to the solicitation will be most advantageous to the Government, price and other factors considered. Award will be made on the basis of the low price technically acceptable quote. EVALUATION STANDARDS. The quoter's technical quote will be evaluated using the following standards for a final rating: ACCEPTABLE: A quote that meets the Government's Technical Factors identified in the solicitation. UNACCEPTABLE: A quote that fails to meet the Government's Technical Factors identified in the solicitation. SPECIAL NOTE: All vendors must be registered in the System for Award Management (SAM) prior to award, and lack of registration shall make an offer ineligible for award.
