Description
The Sierra Army Depot intends to enter into a sole source contract under Simplified Acquisitions Procedures with Specialty Health, 330 E Liberty St Suite 200 Reno 89501 for the primary program components assumed necessary to implement the Employee Fitness and Wellness Program. Comprehensive Health Risk Assessment Web-based Health Risk Assessment Biometric assessment (to include height, weight, blood pressure, waist circumference, nutritional and social habits) Nutrition Counseling: Provision of scientific information and coaching for healthy goals; Stress Reduction: Limiting physical and mental stress via exercise and nutrition; Weight Control: Providing information, support, and programs to help reach healthy body fat goals; Chronic Illness Screening (no medical laboratory requirements): Evaluation and measurement for chronic illness potential and clinical recommendation. The types of chronic illnesses for screening, and the reasoning for such recommendations, at the beginning of the contract term. Worksite Exercise and Education Centers (2 locations) SIAD Fitness Center, Sierra Army Depot currently operates one (1) employee exercise facility on-site. The contractor shall perform the following: Consult and advise program participants on-site during the specified operation house on proper use of fitness equipment, and benefits of exercising related to participant's needs and health conditions. Facilitate and conduct healthy lifestyle education and training sessions at the exercise center and/or appropriate conference room settings that emphasize employee fitness and wellness, and coordinate with the Sierra Army Depot program designee to meet with employees in pre-scheduled group and individual sessions to coach exercises, and provide education on positive behavior change and healthy eating. Therapeutic Program Plan conduct and coordinate individualized exercise programs for each participant, suggested by the proposer and pre-approved by the Sierra Army Depot Health Clinic. These classes will be conducted on an on-going basis during the contract term at the pre-determined locations agreed upon by both parties. The contractor will work with the Sierra Army Depot program designee to promote program benefits and enhance employee's awareness, involvement, participation, and facility utilization. Plan coordinate and conduct no less than four regularly scheduled information seminars on wellness related health topics each contract year, a total of no less than four seminars during the contract term. These seminars will be no shorter than one-hour in length and will be held at various Sierra Army Depot facility sites, with a mutually agreed upon schedule. The final determination will be made by the Sierra Army Depot Health Clinic. Incorporate program participants feedback to modify, update, and adjust class and/or seminar type, size, frequency, or locations to meet the overall program goal of improving employee satisfaction and health condition. Program Monitoring and Data Management The contractor shall perform the following: Provide and utilize a database management system that will provide the necessary technical functions and meets the needs of the program monitor efforts. Track and collect program data on an ongoing basis, maintain and store program data on a centralized computer database system at a contractor's secure site to preserve accuracy, consistency and confidentiality. Provide program data consolidation and program progress analysis, prepare and submit program performance measurement reports to Sierra Army Depot Health Clinic on a quarterly and annual basis. Prepare and distribute outreach materials to advocate the program and to communicate with Sierra Army Depot employees on the program benefits, success, and progress on a regular basis. Collect and analyze employees' feedback on the existing training program, propose and establish a full range of training and class offerings on an on-going basis to accomplish the program's goals. Monitor participant attendance and facility usage frequency on an on-going basis, and maintain this usage in a database. Coordinate with Sierra Army Depot Health Clinic on staff scheduling and facility operational coverage for the program facilities. Medical Professionals. The selected contractor shall provide the following medical professional services: Physician consult support: services shall include review of HRA, lab results, and oversight of nutritional/exercise plans. Dietician available for in-person, e-mail, and phone consultative support to develop and oversee individualized nutrition planning for participants Physical Trainer available for in-person, e-mail, and phone consultative support to develop and oversee individualized exercise planning for participants Laboratory support for the processing of participant lab specimens
