This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Remove all old mattresses from facility Hours 1. Work will be performed during business hours, Monday - Friday, 6am - 4pm. With assistance from VA staff 2. Contractor must be mindful of patients at the VA Medical Center 3. Area must be cleaned and free of any trash or debris left as a result of installation. ATTACHMENT 2: REQUEST FOR JUSTIFICATION AND APPROVAL FOR OTHER THAN FULL AND OPEN COMPETITION DEPARTMENT OF VETERANS AFFAIRS Justification and Approval For Other Than Full and Open Competition 1. Contracting Activity: Department of Veterans Affairs, Rocky Mountain Acquisition Center, VISN 19, on behalf of the Eastern Colorado Health Care System (554), Justification for Other than Full and Open Competition, Brand Name Justification. 554-14-3-149-0041 2. Nature and/or Description of the Action Being Processed: Direct replacement and Purchase of 6 Stryker Isolibrium ICU services that interface with our existing bed frames for Eastern Colorado Health Care System. 3. Description of Supplies/Services Required to Meet the Agency's Needs: 6 Stryker Isolibrium ICU mattresses with Pulmonary rotation and low airloss. Item# 999226619 Manufacture item# PCREVIVEINCMPORAFMT Required Delivery Date: Earliest possible or by 10/30/14 Specifications: -The Isolibrium is a patented specialty mattress with Dartex cover that is only available through Stryker or their third party distributor American Medical Depot -Isolates pressure redistribution through its exclusive air pod technology, which allows both immersion and envelopment, and adds additional contact points to help reduce tissue interface pressure. Uses pod instead of traditional deep cell bladders which decreases bottoming out of a cell. -Four zone sensor technology that automatically adjusts POD pressure while taking into account the patient's weight range, position changes, and frame articulation changes. Mattress adjusts to maintain pod pressure in all frame positions to eliminate bottoming out. -Efficient low air loss delivery from the bottom of the surface in between pods is superior to traditional low air loss delivery. The combination of the cover, air channels, and open pod technology allows air to flow freely throughout the surface without obstruction. -Customization of the rotation therapy based on patient tolerance. When the head of the bed is elevated to 30 degrees a full rotation can still be achieved. Due to length of the turn bladder, the pods and pillow adjusting, a full body properly aligned turn is achieved when using lateral rotation and turn assist. Foam crib provides better shoulder support for proper body alignment during the turn. 4. Statutory Authority Permitting Other than Full and Open Competition: ( x) (1) Only One Responsible Source and No Other Supplies or Services Will Satisfy Agency Requirements per FAR 6.302-1; Isolibrium surfaces are patented products that are only available through American medical Depot who own exclusive rights to sell this Stryker product. As this product must interface with existing equipment, no alternative options are available. 5. Demonstration that the Contractor's Unique Qualifications or Nature of the Acquisition Requires the Use of the Authority Cited Above (applicability of authority): a. Contractor owns the exclusive rights to sell Stryker surface products. Product must interface with existing frame 6. Description of Efforts Made to ensure that offers are solicited from as many potential sources as deemed practicable: As this company owns the rights to distribute Stryker products and the isolibrium is the replacement for our current mattresses with capability to interface with our current ICU frames. There is not another option at this time. 7. Determination by the Contracting Officer that the Anticipated Cost to the Government will be Fair and Reasonable: 8. Description of the Market Research Conducted and the Results, or a Statement of the Reasons Market Research Was Not Conducted Limited due to patented products, direct replacement need, and interface with current products. Market research was also limited by the exclusive rights to sell this product only being available through one vendor. 9. Any Other Facts Supporting the Use of Other than Full and Open Competition: None, see paragraphs above. 10. A Statement of the Actions, if any, the Agency May Take to Remove or Overcome any Barriers to Competition before Making subsequent acquisitions for the supplies or services required: None vendor has exclusive selling rights to the needed product. No comparable product with POD technology available on the market that interfaces with existing equipment. 11.Requirements Certification: I certify that the requirement outlined in this justification is a Bona Fide Need of the Department of Veterans Affairs and that the supporting data under my cognizance, which are included in the justification, are accurate and complete to the best of my knowledge and belief.
