- This is a sources sought notice to determine the availability of potential sources having the skills and capabilities necessary to provide artificial limbs for the veteran beneficiaries of the VA Medical Center in Erie, Pennsylvania. All interested vendors are invited to provide information to contribute to this market survey/sources sought including commercial market information. THIS IS NOT A SOLICITATION ANNOUNCEMENT. This is a sources sought synopsis only. The purpose of this synopsis is to gain knowledge of potential qualified sources and their size classifications (Service Disabled/Veteran Owned Small Business (SDVOSB/VOSB), Hub zone, 8(a), small, small disadvantaged, woman-owned small business, FSS/GSA contract schedule holders or large business) relative to NAICS 339113 -Surgical Appliance and Supplies Manufacturing, Size Standard 500 Employees. Responses to this synopsis will be used by the Government to make appropriate acquisition decisions. After review of the responses to this sources sought synopsis, a solicitation announcement may be published on the FBO website. Responses to this sources sought synopsis are not considered adequate responses to the solicitation announcement. All interested offerors will need to respond to the future solicitation announcement. For purpose of this Sources Sought Request for Information, provide the information requested below. The VA Medical Center - Erie, Pennsylvania is seeking sources for a potential contractor to provide artificial limbs to its veteran beneficiaries. Interested sources are required to submit an e-mail delineating qualifications, capabilities, and experience for providing artificial limbs. Responses to this notice should include the following: 1.Company Name. a.Person of Contact, Address, Telephone Number, Email Address b.DUNS Number 2.Socio-economic status (e.g. SDVOSB, VOSB, Woman-Owned, etc.). 3.Do you currently hold any contracts providing these services (federal, state, local agency or private facility)? If so, include facility, contract number, dollar value, points of contact, email addresses, telephone numbers of individuals that can verify this information. 4.Include past experience in performing these services to the VA, other Government (Federal or State) agency, or for a private facility. Include facility, contract number, dollar value, points of contact, email addresses, telephone numbers of individuals that can verify this information 5.Are you available to provide services throughout the Erie VAMC Catchment Area? The Erie VAMC services veterans in the following counties: a.COUNTIES IN PENNSYLVANIA: Erie, Crawford, Forest, McKean, Mercer, Venango, and Warren b.COUNTIES IN OHIO: Ashtabula c.COUNTIES IN NEW YORK: Chautauqua 6.What is the timeframe required to initiate service? 7.What is your companies experience in providing similar services which demonstrates firm's ability to meet the requirement? Responses must be emailed to Cynthia.grygier@va.gov no later than 2:00PM EST, Friday, August 29, 2014. Your response should include both the STATEMENT OF CAPABILITY and BUSINESS SIZE AND SOCIO-ECONOMIC STATUS information as explained above. This notice is to assist the VA in determining sources only. A SOLICITATION IS NOT CURRENTLY AVAILABLE. If a solicitation is issued it will be announced at a later date, and all interested parties must respond to that solicitation announcement separately from the responses to this announcement. Draft Scope of Work The contractor shall provide artificial limbs and related services to the veteran beneficiaries of the Erie VA Medical Center (Erie VAMC), 135 East 38th Street, Erie, Pennsylvania. It is the intent of the Government to award a firm fixed price, indefinite delivery/indefinite quantity contract. The Government reserves the right to award multiple contracts. This contract will be for a period of one (1) year from date of award plus four (4) additional one year options. The contractor shall fabricate all artificial limbs in strict conformance to the prosthetic prescription that has been provided by the Erie VAMC Prosthetic Clinic. The Contractor's facilities must be approved by the Facility Accreditation Program of the American Board for Certification in Orthotics and Prosthetics, Inc. (ABC) or the Accredited Facilities Program of the Board of Certification (BOC). The Contractor shall have a full-time American Board Certified (ABC) or Board for Orthotist-Prosthetist (BOC), or VA Qualified (VAQP) Prosthetist on staff at each of their locations in good standing. The fitting and alignment of the following prostheses/services must be performed under the supervision of a Prosthetist who has met the applicable education, training, and licensing requirements: (1) Ischial Containment Prosthetic Designs (2) Microprocessor knee (3) Silicone suction sockets (3S) (4) Tec Harmony System The training of the Prosthetist shall include Computer Aided Design /Computer Aided Manufacture (CAD/CAM), Longs Technique, National Student Nurses Association (NSNA), and/or educational seminars specific to the fabrication and servicing of these components that were presented by organizations such as National Association for the Advancement of Orthotics and Prosthetics (NAAOP), American Academy of Orthotics and Prosthetics (AAOP), American Orthotic and Prosthetic Association (AOPA), International Association of Orthotics and Prosthetics (IAOP). VA shall require the Contractor to deliver items ordered under this contract no later than 30 calendar days after receipt of a purchase/delivery order. Quotes must be returned to the requestor within 10 days of receipt of request for quote (RFQ) and an itemized list by L-Code. The Contractor warrants the artificial limb/brace provided against defective material, workmanship and fit for a minimum of 90 calendar days from the date of acceptance. The Contractor shall immediately notify all effected VAMC (s) and contracting activities in the event of a product recall, removal, required adjustment, or required modification that is suggested or mandated by the Contractor, distributor, manufacturer, or any regulatory or official agency. The Contractor agrees that by accepting an Erie VAMC patient for care, it has available sufficient qualified staff capable of providing quality care necessary to meet the vision of the Erie VAMC, which is to honor American veterans by providing exceptional healthcare that improves their health and well-being. To assure continuity of care for our patients, the following timelines and customer service standards will be adhered to when treating EVAMC patients (Reference VHA Directive 2006-041, Veterans Health Care Service Standards, dated 6/27/06) "Patients will be seen within 20 minutes of scheduled appointment time. "Patients will have an appointment with a specialist, when referred from EVAMC, within 30 days of referral.
