Description
The Department of Veterans Affairs VIN 15 Medical Centers seeks a single or multiple award contracts for Home Oxygen and Ventilator delivery services. This is a Total Small Business set aside acquisition. All interested offerors should furnish all labor, supervision, equipment, training, and supplies to provide Home Oxygen and ventilator delivery services. VISN 15 has approximately 4,454 beneficiaries on home oxygen and 10 patients on ventilators as of January 2014. The total number of patients fluctuates each month due to patients being added/removed from the program. Records show that the overall number of patients on home oxygen has increased 25% from January 2009 (3576) to January 2014 (4,454). The contract shall be for the actual requirements of the VA as ordered by the VA during the life of the contract. The Contracting Officers Representative (COR) or designee will provide the contractor with notification including date and place of delivery to initiate individual beneficiary supply and service requirements, including the beneficiary's oxygen prescription, equipment, supplies and training services for veteran beneficiaries. All services shall be performed in the veteran's place of residence. The area of service shall be all patients serviced by VISN 15, including catchment areas, regardless of where Patient resides. The equipment needs are subject to change as determined by the prescribing physician. Contractors shall have a facility or resources physically located in the geographic area (35 miles radius of each medical center) in which they shall provide service. Non-territory requests shall be required from time to time and the Contractor shall accept these "non-territory" veterans at that Contractor's awarded price. VA reserves the right to assign these Patients to any contractor. The geographical boundaries described herein have been determined as the areas of responsibility/jurisdiction for each local VA Medical Center (see VISN 15 Facilities and Counties covered attached: Kansas City VA Medical Center (VAMC) 801 Linwood Blvd Kansas City, MO 641 28Harry S. Summary of Respiratory Related Equipment/Services and Delivery, Instruct and Retrieval to be solicited: The Contractor shall provide ongoing delivery, set-up and education and follow-up to 4200 existing and all ongoing new patient set ups for rental oxygen systems and respiratory related equipment and supplies. This includes storage and maintenance of all high-pressure cylinders, concentrators, oxygen refill systems, ventilators, emergency back-up systems and supplies to eligible VA beneficiaries in their place of residence as required by the hospital systems. The contractor should also have the ability to arrange travel for veterans outside of the service area, in coordination with other vendors at the point of destination. It is recommended that the Contractor will have one Full Time Respiratory Therapists per VA site to provide ongoing delivery, set-up, education and follow-up of equipment and supplies to patients. They will also provide all follow-up visits for new oxygen set ups and triage and provide service and follow-up to our high risk patient population. The therapist will also serve as the primary point of contact for clinical questions, care coordination, and expediting travel that arise from time to time and will provide support for the local VA Prosthetics Home Respiratory Coordinator. Services provided shall be all inclusive of delivery/set-up of equipment, in-service and ongoing training for beneficiaries/ care providers to include patient education, periodic safety inspections and Preventive Maintenance Inspection (PMI) service appointments to provide safety and ongoing equipment education every 6 months for all oxygen recipients with 100% compliance to these visits mandated. All new set ups or orders to the vendor to provide service and/or equipment shall be provided per prescription within the geographic location specified in the solicitation. This will include, but not limited to: hospitals, veteran's residence, or in a nursing home where the veteran resides. The Prosthetics service/Home Respiratory Coordinators specifically authorized in the contract (hereafter authorized personnel), will notify the Contractor, via fax, verbally or email (if applicable) to initiate service; the beneficiary's oxygen prescription, the equipment, supplies and services to be provided, including date and place of delivery. NAICS Code 621610 (size standard of $14.0 Million) and PSC Code Q201 are applicable to this acquisition.
