Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6-streamlined procedures for commercial items, in conjunction with FAR subpart 13 and as supplemented with additional information included in this notice. This is a Firm Fixed Priced Contract. Only qualified offerors may submit offers. Solicitation VA-249-14-Q-0838 is issued as a Request for quote (RFQ) and constitutes the only solicitation, a written solicitation will not be issued. This procurement will be a full and open competition. NAICS Code is 334290 and size standard is 750 employees. Any firm that does not meet the capability and size standard under this NAICS code should not submit a response to this notice. In order to be considered for a government award, the firm must be registered in SAM @ www.sam.gov and be capable of providing all parts, service, manuals, tools or software required to successfully complete installation of Bio reader equipment listed in the attached statement of work. This service is required for the Nashville and Murfreesboro Tennessee VA Medical Centers, at 1310 24th Ave South Nashville TN 37212 and 3400 Lebanon Pike Murfreesboro, TN within VISN 9, in accordance with the terms, conditions, special provisions and schedules herein. In order to be responsive, the firm must offer a quote on all itemized items and be authorized to sell the specified equipment and provide all items without substitution. Award will be made to the lowest price quote which conforms to the requirements within this solicitation. Submitting Quote: Contractor shall submit their quote on company letterhead and shall include the completed pricing forms, along with name, address, telephone number of the offeror, firm's DUNS#, overall total price and a completed copy of Clause 52.212-3 Offerors Representations and Certifications- Commercial Items or vendor's ORCA document. DESCRIPTION/SPECIFICATION/WORK STATEMENT Install Pharmacy PIV Bio Readers TVHS PACS STATEMENT OF WORK PART A - GENERAL INFORMATION A.1 INTRODUCTION - This requirement is to provide and install12 iClass PIV Bio readers connecting the TVHS Pharmacy doors and Vaults to the TVHS C-cure 9000 PACS system. The installation of this equipment will allow for authorized Pharmacy Staff to use their PIV cards in conjunction with biometric identification for secure Pharmacy access control. A.2 BACKGROUND - There is an ongoing VACO Physical Access Control System (PACS) project in process at the VHA TVHS Nashville and Murfreesboro Campuses. This work is an add to the already in progress VACO project. This add cannot be funded by the VACO project and the VACO project cannot be amended or changed to provide this work TVHS required work. A.3 SCOPE OF WORK - "Contractor shall provide all labor, supplies, tools, equipment, programming, personnel and testing required to connect 12 MorphoAccess SIGMA (MA SIGMA) Series PIV Bio readers to the TVHS Pharmacy doors and Vaults and to the TVHS C-cure 9000 PACS as a complete and working system. "Work will be performed at both the TVHS the Nashville and Murfreesboro main Campuses STATEMENT OF WORK PART C - SUPPORTING INFORMATION C.1 Place of Performance -VHA TVHS Pharmacy's, Nashville and Alvin C York Campuses. C.2 Period of Performance - Contract period is from date of award until all work completed Contractor shall perform work during normal work hours which are 8:00 a.m. until 4:30 p.m. Monday through Friday.
