Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation VA251-14-Q-0429 is issued as a request for quotes (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-12. The North American Industry Classification System (NAICS) number is 339113 and the business size standard is 500 employees. This is a small business set aside. All responsible sources may submit a proposal which shall be considered by the contracting officer. Requested Items: SPECIFICATIONS/STATEMENT OF WORK Veterans Affairs Medical Center Northern Indiana has a need to purchase four (4) Portable Light Tower/Generators for emergency police operations at geographically separated areas located in the Northern Indiana area with delivery to the Marion Indiana facility located at 1700 East 38 Street, Marion, Indiana 46953. 1.QUALIFICATIONS The contractor shall meet all certification, registration and licensure requirements of Federal, State or Local laws regarding operation of this type of service. 2.NEEDED RESOURCES a. Four (4) Portable Light Tower/Generators b. ORDERS Vendor will ship to Northern Indiana Health Care Police Service 1700 East 38th Street Marion, Indiana 46953. SPECIAL CONTRACT REQUIREMENTS 5.SERVICES a. The services specified in the sections entitled Schedule of Supplies/Services and Special Contract Requirements may be changed by written modification to this contract. The VA Contracting Officer will prepare the modification. b. There is no employer-employee relationship between the VA and the contractor or the contractor's employee(s). 6.TRAINING a. The contractor will be responsible to ensure police employees are fully trained and completely competent to perform operate the installed systems whether by classroom or classroom supplied training material. b. The VA COTR will monitor the contractor employees' work to ensure contract compliance. c. Notwithstanding other contract requirements, upon request of the Contracting Officer, the contractor will remove any contractor employee who does not comply with orientation requirements or meet competency requirements for the work being performed. d. When changes in contractor personnel are approved in accordance with the "Key Personnel" clause of the contract, the contractor must provide evidence of orientation, the current competence assessment, and current performance evaluation that supports the above. 7.HHS/OIG To ensure that the individuals providing services under the contract have not engaged in fraud or abuse regarding Sections 1128 and 1128A of the Social Security Act regarding federal health care programs, the contractor is required to check the Health and Human Services - Office of Inspector General (HHS/OIG), List of Excluded Individuals/Entities on the OIG Website (www.hhs.gov/oig) for each person providing services under this contract. Further the Contractor is required to certify in its proposal that all persons listed in the contractor's proposal have been compared against the OIG list and are NOT listed. During the performance of this contract the Contractor is prohibited from using any individual or business listed on the List of Excluded Individuals/Entities. [End of Special Contract Requirements] 8.EMERGENCIES An emergency, for the purposes of this contract, is defined as any unforeseen circumstance or situation which requires immediate action by the Contractor. Examples of emergencies include, but are not limited to, unusually severe weather, natural disasters, any situation which may affect the safety and security of VA patients, employees, contractor employees, or other individuals, or any other situation or event as identified by the COTR or his/her designee will provide direction to the Contractor with respect to any and all identified emergencies. Items will be shipped to the VA Medical Center, 1700 East 38th Street, Marion, IN 46953-4568.
