This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (1) The solicitation number VA259-14-Q-0707 issued as a request for quotation (RFQ). (2) This solicitation is set-aside for 100% Small Business under NAICS code 337214. (3) Contract Line Item number:7 (4) Description of requirements for the items to be acquired: (see attached SOW line items descriptions for price submission) (5) Delivery: 15 Days ARO, FOB Destination, Deliver To: VA Health Care System Bldg 7 Warehouse 500 Foothill Drive Salt Lake City, UT 84148-0001 (6) Clause 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition. (7) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: Product shall meet or exceed the specifications listed in the line item. Any substitution must provide detailed product descriptions and justification as to how the product deemed met or exceeded. No partial quote or shipment will be accepted. (14) Offer, in writing, due by close of business 29 August 2014 (15i) Inquiry and offer submission shall be addressed/submitted to the point of contact: Silentip Tubtim, Silentip.tubtim@va.gov STATEMENT OF WORK (SOW) Anthro Tables and Chairs The VA Salt Lake City Healthcare System (VASLCHCS) has an ever increasing patient load. Radiology is one of the departments increasing its workload daily. The volume of scans that have to be read by a physician has also increased; a physician can spend more than eight hours in the dark reading room in just one day. This increases workplace injuries to the neck, back, legs, arms, and wrists. In an effort to decrease workplace injuries and enable the physician to be more productive in reading critical scans the VASLCHCS is replacing all of the current tables and chairs with a more ergonomic reading room environment. The Anthro chair supports the head, neck, shoulders, and forearms to reduce strain on the body. A series of eleven torsion spring-loaded bearing joints conform to the exact spine impression of the physician. Dimensions: Seat measures 21"w x 19.5"d. Back measures 19.5"w x 30"h. Height range 18"-21"h. The Anthro table has a powered 3-port USB hub to provide desktop connectivity for all devices. Twin dimmable LED task lights, cooling fans that keep air moving in front of the displays. The table is adjustable to accommodate physicians sitting or standing. Specifications: -CT12 adjusts in height 30"-49.5" high on electric lift legs. The table also tilts 0-15 degrees. -With up to 100 programmable preset positions, you can record your favorite work position then just enter your code when you sit down to work. The table will automatically adjust to your settings. -The unique Monitor Control System adjusts the height and depth of the monitor bar with push-button control. Comes with installation points for up to 3 monitors; easily upgrades to support up to 5 monitors. -An adaptive lighting system uses a sensor to automatically adjust the ambient light in the workstation to the image on the screen to help reduce your eye fatigue. Two task lights illuminate the work surface. -A Privacy Panel shields you from distractions, reduces glare, and diminishes noise up to 35%. -The Air Circulation System has two adjustable vents at each side of the table to keep gentle air flowing around you, refreshing you while you work. -The table's safety system stops the table's motion when it contacts an obstacle. -The easily-accessed powered USB Hub allows you to plug in up to 3 peripherals. -The large keyboard surface (57.75"w x 23.5"d) provides plenty of space for keyboard, mouse, and documents. The cushioned surface is comfortable against your arms and keeps items in place when the table is tilted. -As part of the package, a knowledgeable Field Service Engineer travels to your site to install the table and show your staff how to use it. -Tested and certified for safety to UL standard 60950. -Comes with 2 electrically controlled shelves (1 monitor, 1 keyboard) with height and tilt adjustments, side shelf, back Privacy Panel, 2 height-adjustable legs, CPU holder, footrest, 2 background lights, 2 LED task lights, 2 control panels, powered USB hub, Air Ventilation System with 2 adjustable air ports, Monitor Control System with 3 monitor brackets and cable management troughs, 3 power strips with 14 outlets, and a 10' power cord with cable management sleeves. -Lifetime warranty against manufacturing defects on the table; 2-year warranty on the electrical.
