Description
Purpose and Objectives: The intent of this Sources Sought Notice is to verify and confirm potential offerors who can supply: High-Low Treatment Tables/ Digital Wheelchair Ramp Scales 334510ELECTROMEDICAL AND ELECTROTHERAPEUTIC APPARATUS MANUFACTURING / 500 Employees The vendor/contractor will provide 6 high low treatment tables and 3 digital wheelchair ramp scales as described below. Equipment will be maintained and checked in by Atlanta VA Medical Center biomedical engineer department. No installation is required. SALIENT CHARACTERISTICS Line Item #1 Treatment tables: A.The high-low treatment tables must be able to comfortably accommodate patients ranging from height 5'- 6' tall and weighing 120 lbs. to 400 lbs. B.The treatment tables need to have three pieces with head section. C.Must be safe to be wipe-down and clean with germicidal detergent. D.Table needs to have electrical adjustment capability, by foot is preferred for the provider. E.The vendor or manufacturer must offer a warranty of at least 1 year for parts and service. Line Item #2 Digital wheelchair ramp scale: A.The digital wheelchair ramp scale must be accessible to patients who suffer from a variety of disabilities, such as patients with limited movement of extremities, general frailty, paraplegics and wheelchair users. B.The scale needs to have a ramp to allow easy wheelchair access to the scale platform. C.The scale must be able to accommodate weight up to 1, 000 pounds. D.The vendor or manufacturer must offer a warranty of at least 1 year for parts and service. DELIVERY Vendor or carrier transport must be able to deliver items to Atlanta VAMC loading dock, 1670 Clairmont Road Decatur, GA 30033, within 30 days of contract awarded. Equipment should meet all criteria listed in SOW. Response: Responses to this Sources Sought Notice should demonstrate the firm's ability, capability, and responsibility to provide supplies listed above. Responses should include the following information: Authorized Distributor and/or Exclusivity Letter, Business name, address, business size and Socioeconomic Category (HUBZone, 8(a), Service Disabled Veteran, Small Business, etc.), and point of contact to include e-mail address and telephone number. All information is to be submitted via e-mail to Tanjia.Harris3@va.gov. Information provided will not be returned. All responses shall be in the English Language. Responses are due by 6:00 pm (EST) on Monday, August 25, 2014. No submissions will be accepted after this date and time. Questions can be submitted electronically to Tanjia.Harris3@va.gov. This is a Sources Sought Notice and submissions will be used for informational and planning purposes only. This notice does not constitute a formal Request for Quote (RFQ), nor is the government obligated to issue an RFQ. In addition, the Government does not intend to pay for any information provided under this notice. The Government is not obligated to notify respondents of the results of this survey.
