- Performance Work Statement Request for Quote Conbined Synopsis-Solicitation Response to this combined synopsis/solicitation must be received via email, fax, mail, or delivery by 12SEP2014 no later than 2:00 PM Pacific Standard Time. Requests should be marked with solicitation number FA4686-14-T-0035. COMBINED SYNOPSIS - SOLICITATION FOR COMMERCIAL ITEMS (IAW FAR 12.603 (c)) Requirement: Heavy Duty Industrial Shelving & Install (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Proposals are requested and a written solicitation will not be issued. (ii) This solicitation/synopsis reference number is FA4686-14-T-0035 and is being issued as a Request for Quote (RFQ). (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 29 May 2014. (iv) This acquisition is small businesses set-aside. The associated NAICS code is 337215 with size standard of 500 employees. (v) Contractors shall submit a lump sum quote and fill out the below RFQ information (with the break-out of CLIN's) for heavy duty industial shelving and install. All responsible sources may submit a quotation, which shall be considered. (vi) Requirements for heavy duty industrial shelving and insall services on Beale Air Force Base, California are outlined in the Statement of Work. (See attached statement of work) (vii) Period of Performance: NET 30 after award (viii) The following clauses and provisions are incorporated and will remain in full force in any resultant award: (full text of clauses and provisions may be accessed electronically at this website: http://farsite.hill.af.mil) FAR 52.212-1, Instructions to Offerors-Commercial (ix) FAR 52.212-2 - Evaluation - Commercial Items (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be the best value to the Government, price and other factors considerd. The following factors shall be used to evaluate offers: Lowest price, Technically Acceptable. (b) Initially, offers will be listed according to price. When determining lowest price, the Government may use the total price of individual contract line items or of the entire offer. If determined to be in the Government's best interest, more than one award can result from this solicitation. The lowest priced offeror will be evaluated for technical acceptability. Technically acceptable is defined as meeting the salient characteristics of the item(s) requested. If determined to be technically acceptable, award will be made to that offeror. If not technically acceptable, the next lowest priced offeror will be evaluated in the same manner. The process will continue until the lowest priced technically acceptable offer is found. (c) Past Performance - The offeror shall submit a listing of relevant past contracts pertaining to its experience over the past three (3) years in performing mission critical functional and support services as described in the PWS. The items in said list shall be both recent and relevant. The list shall contain the contract number, dollar value, description of the project, Government points of contact (name, address, telephone number and email address) and cite any challenges encountered and overcome. "Recent" is defined as previously performed this type of work within the past three (3) years. "Relevant" is defined as work that is the same or very similar to the work being identified in the PWS. The Past Performance Questionnaire (PPQ), provided in this solicitation, shall be sent by the offeror to their customers to allow for sufficient time for completion and inclusion in the offeror's proposal. Past Performance addressed in the PPQ shall be both recent and relevant. The Government will evaluate whether the offeror and/or proposed personnel (if applicable) demonstrate a successful performance history in meeting or managing the same or similar technical requirement and the currency of their performance history. The PPQ is a method of collecting this information however, past performance information may be obtained from any other sources available to the Government to include, but not limited to, CPARS, PPIRS, EPLS or other databases, interviews with Program Managers, Contracting Officers and other Government representatives. This factor has no page limit. Past Performance Acceptable/Unacceptable Ratings Past Performance Evaluation Ratings Description Acceptable Based on the offeror's performance record, the Government has a reasonable expectation that the offeror will successfully perform the required effort, or the offeror's performance record is unknown. To receive an acceptable rating, past performance must also comply with the definitions of "recent" and "relevant". Unacceptable Based on the offeror's performance record, the Government has no reasonable expectation that the offeror will be able to successfully perform the required effort. Unacceptable means that the past performance doesn not meet the definitions of "recent" and "relevant". Note: In the case of an offeror without a record of relevant past performance or for whom information on past performance is not available or so sparse that no meaningful past performance rating can be reasonably assigned, the offeror may not be evaluated favorably or unfavorably on past performance (see FAR 15.305 (a)(2)(iv)). Therefore, the offeror shall be determined to have unknown past performance. In the context of acceptability/unacceptability, "unknown" shall be considered "acceptable." Rating Description Acceptable Proposal clearly meets the minimum requirements of the solicitation. Unacceptable Proposal does not clearly meet the minimum requirements of the solicitation. (d) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). (e) Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award. The Government does not intend to conduct negotiations but reserves the right if determined to be in the best interest of the Government. (x) All offerors shall include a completed copy of FAR 52.212-3, if the offeror has completed the annual representation and certifications electronically via http://www.acquisition.gov at the System of Award Management (SAM) website. (xi) The clause at 52.212-4 Contract Terms and Condition-Commercial Items, applies to this acquisition. As used in this clause- (1) "Contract financing payment" and "invoice payment" have the meanings given in section 32.001 of the Federal Acquisition Regulation. (c) The Contractor may submit a payment request and receiving report using other than WAWF only when- (1) The Contracting Officer administering the contract for payment has determined, in writing, that electronic submission would be unduly burdensome to the Contractor. In such cases, the Contractor shall include a copy of the Contracting Officer's determination with each request for payment; (2) DoD makes payment for commercial transportation services provided under a Government rate tender or a contract for transportation services using a DoD-approved electronic third party payment system or other exempted vendor payment/invoicing system (e.g., PowerTrack, Transportation Financial Management System, and Cargo and Billing System); (3) DoD makes payment for rendered health care services using the TRICARE Encounter Data System (TEDS) as the electronic format; or (4) When the Governmentwide commercial purchase card is used as the method of payment, only submission of the receiving report in electronic form is required. (d) The Contractor shall submit any non-electronic payment requests using the method or methods specified in Section G of the contract. (e) In addition to the requirements of this clause, the Contractor shall meet the requirements of the appropriate payment clauses in this contract when submitting payment requests. • DFARS 252.246-7000, Material Inspection And Receiving Report • AFFARS 5352.223-9001, Health and Safety on Government Installations, (a) In performing work under this contract on a Government installation, the contractor shall: (1) Take all reasonable steps and precautions to prevent accidents and preserve the health and safety of contractor and Government personnel performing or in any way coming in contact with the performance of this contract; and (2) Take such additional immediate precautions as the contracting officer may reasonably require for health and safety purposes. (b) The contracting officer may, by written order, direct Air Force Occupational Safety and Health (AFOSH) Standards and/or health/safety standards as may be required in the performance of this contract and any adjustments resulting from such direction will be in accordance with the Changes clause of this contract. (c) Any violation of these health and safety rules and requirements, unless promptly corrected as directed by the contracting officer, shall be grounds for termination of this contract in accordance with the Default clause of this contract. • AAFARS 5352.242-9000, Contractor Access to Air Force Installations. (a) The contractor shall obtain base identification and vehicle passes, if required, for all contractor personnel who make frequent visits to or perform work on the Air Force installation(s) cited in the contract. Contractor personnel are required to wear or prominently display installation identification badges or contractor-furnished, contractor identification badges while visiting or performing work on the installation. (b) The contractor shall submit a written request on company letterhead to the contracting officer listing the following: contract number, location of work site, start and stop dates, and names of employees and subcontractor employees needing access to the base. The letter will also specify the individual(s) authorized to sign for a request for base identification credentials or vehicle passes. The contracting officer will endorse the request and forward it to the issuing base pass and registration office or Security Forces for processing. When reporting to the registration office, the authorized contractor individual(s) should provide a valid driver's license, current vehicle registration, and valid vehicle insurance certificate to obtain a vehicle pass. (c) During performance of the contract, the contractor shall be responsible for obtaining required identification for newly assigned personnel and for prompt return of credentials and vehicle passes for any employee who no longer requires access to the work site. (d) Upon completion or termination of the contract or expiration of the identification passes, the prime contractor shall ensure that all base identification passes issued to employees and subcontractor employees are returned to the issuing office. (e) Failure to comply with these requirements may result in withholding of final payment. • AFFARS (xiv) Response to this combined synopsis/solicitation must be received via email, fax, mail, or delivery by 12SEP2014 no later than 2:00 PM Pacific Standard Time.
