- RFQ package which included SF-30, specifications, clauses. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. FAR Part 12, Acquisition of Commercial Items (Title VIII of the Federal Acquisition Streamlining Act of 1994) (Public Law 103-355) & FAR 37.4, Non-personal Health Care Services Contract (10 U.S.C. 2304 and 41 U.S.C.253). This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The Great Plains Area Indian Health Service (IHS) intends to award a firm fixed-price, commercial item, purchase order in response to Request for Quote RF-241-14-097. The solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-76. The associated North American Industry Classification System code is 335122 and the Small Business size standard is 500 employees. THIS SOLICITATION IS ISSUED AS 100% SMALL BUSINESS SET-ASIDE. The quoted unit pricing must be an all inclusive cost (to include but not be limited to transportation, food, lodging, per diem, federal, state and local taxes), plus all other costs pertinent to the performance of this contract. Utilize your most competitive and reasonable rates. LED Lighting for the Cheyenne River Health Center, Eagle Butte, South Dakota, supplies be delivered to the service unit within 45 days after award. Please refer to the attached SF-18 and specifications for the light fixtures. Period of performance: This contract will be from "Date of Award" and delivery within 45 day from the award. BASIS FOR AWARD: Lowest Price Technically Acceptable Brand Name or Equal FAR 52.212-1 - Instructions to Offerors - Commercial Items (February 2012): Quotes shall be submitted on company letterhead stationery, signed and dated; it shall include: 1. Solicitation number RFQ-241-14-097 2. Closing date of August 29, 2014 at 12:00pm EST 3. Name, address, telephone number of the offeror and email address of the contact person. 4. Terms of any express warranty 5. Price of any discount terms 6. "Remit to" address, if different than mailing address; 7. Acknowledgement of Solicitation Amendments; 8. A completed copy of the representations and certifications at FAR 52.212-3 9. A statement specifying the extent of agreement with all terms, conditions, and provisions included in the solicitation. Offers that fail to furnish required representations or information, or reject the terms and conditions of the solicitation may be excluded from consideration. 10. Contractor will need a DUNS number, TIN number and be registered with www.SAM.gov and submitted with the quote. FAR 52.212-3 - Offeror Representations and Certifications - Commercial Items (April 2012) - See attachment for full text and instruction for completion.. An offeror shall complete only paragraph (b) of this provision if the offeror has completed the annual representations and certifications electronically via http://www.sam.gov. If an offeror has completed the annual representations and certifications electronically at the System for Award Management (SAM) website, the offeror shall complete only paragraph (c) through (o) of this provision.
