1.When is the due date for this? September 5, 2014 2.Who is the current company that is providing the services? Medical Logistics Solutions 3.What the amount that was billed out for the previous contract? $161,923.09 4.If this job was performed before, is the current SOW that same that is listed in this new solicitation? Yes 5.What is the approximate number of scheduled deliveries/day/location? Three deliveries per day, Monday through Friday, between the Western Salt Lake Community Based Outpatient Clinic, WSLCBOC and the VA Salt Lake City Health Care System, VASLCHCS. 6.What are the approximate number of on-demand deliveries/month/location? Estimated 5 deliveries per month between the VASLCHCS Pathology and Laboratory and one of these - Utah Public Health Laboratories, Utah Department of Health, American Red Cross. 7.Are Scheduled deliveries required to be delivered by a certain time and if so what are those time requirements? When items are picked up they should be taken directly to the specified location with a drop off/delivery of no later than 12:45 pm and 4:45 pm. 8.Are deliveries daily? 5 days/Week? WSLCBOC deliveries are M-F. The lab deliveries are on-call basis 7 days a week. 9.Are there weekend deliveries? Only for the lab On-Call deliveries. 10.Are pick-ups typically ready by a specific time or are drivers required to wait? Pick-ups are typically ready when the driver arrives. If they arrive early they may have to wait a few minutes for labs. 11.What are the projected pick-up times for each facility on the list? Pick-ups from the VASLCHCS - 8:00 am, 11:00am, 3:15pm for delivery to the WSLCBOC. Pick-ups from the WSLCBOC - 9:00am, 12:00 pm, 4:00 pm for delivery to the VASLCHCS. 12.Do drivers need special security clearances for pick-up or delivery? They will need to get badges for access to the mail cage and they will need PIV badges but no other security clearances.
