Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation, W912L9-14-T-0076, is being issued as a Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-72. This procurement is a 100% Small Business set-aside IAW FAR 19.502-2(a). The NAICS code is 334310 and the small business size standard is 750 employees. The following commercial items (BRAND NAME OR EQUAL) are requested in this solicitation: CLIN 0001: Description: Projector Equipment & Installation Quantity: 1 Unit of Issue: Lot SCHEDULE OF ITEMS (BRAND NAME OR EQUAL) (all quantities are quote mark 1 ea quote mark unless otherwise specified): -NEC DLP projectors, 7000 lumens, 1280 x 800 native (2 ea) -Da-lite Ceiling recessed screen, electric, 87 quote mark x 139 quote mark -Da-lite Fixed frame screen for back of room 69 quote mark x 110 quote mark -Chief Mounting hardware for projectors -Extron Video switcher/scaler, 2 VGA and 4 HDMI inputs -Extron HDMI over cat 6 transmitters (2 ea) -Extron HDMI over cat 6 receivers (2 ea) -Extron Auto switcher, VGA and HDMI over cat 6 -Extron HDMI over Cat 6 receiver -Sony Blu-ray player -Extron 12' VGA w/audio (2 ea) -Extron 12' HDMI cables (4 ea) -Extron 6' HDMI cables (4 ea) -IR extender -Shure Wireless mic, handheld -Shure Wireless mic, bodypack -Countryman E6 mic to terminate with Shure bodypack -Soundcraft 12 channel audio mixer -Shure Audio processor, feedback reduction, graphic eq -Crown Amplifier, 300 watts per channel -JBL 12 quote mark two-way loudspeaker system (2 ea) -Extron Shielded Cat 6 (400 ft) -WestPenn Speaker Wire (200 ft) -Wall plate: XLRF -Wall plate: pass through -Installation and Training -Trip Fee -Shipping PERFORMANCE WORK STATEMENT FORT WAYNE, IN 46809-3158 Contractor Responsibilities Contractor, installation subcontractor, or warranty maintenance contractor must reside within a 2-hour radius of the work site so as to be available for same-day and emergency warranty work and/or maintenance. All material will be staged in a manner which will not prevent the effective use of the facility during all phases of installation. All employees working on-site must submit to 122 FW/SFS a name, DLN with issuing state, and DOB for background checks NLT than 72 hours prior to start of project. All employees will comply with all safety standards, wear proper PPE, and stay within designated work area. Use of Government Owned Equipment is not authorized. Any equipment needed for project must be provided by the contractor, including manlifts, ladders, etc. Provide a set of quote mark as-built quote mark drawings in electronic & paper format. Electronic format must be Visio 2010 compatible or newer. Provide information to Contracting Officer for the staging area for materials. This would include any materials the government would procure and have available on-site as government furnished material. Contractors are responsible to restore the work site to original condition should any damage be incurred by the contractor. Be responsible for the method, procedure, and funding to off-load the Contractor provided equipment/materials GFE at the designated staging area on the site. All cables, panels, patch cords, wall plates, etc. will be labeled according to the customers naming convention and must match to the corresponding drawings. All cable runs will be secured and out of view as much as possible. All cable runs will take the shortest possible route from equipment to system controllers. The use of zip ties and Velcro is the only authorized method of securing lines and will be done in a manner to properly secure all wires. PLANNED INTERRUPTION TO SERVICE shall be coordinated 24 hours prior to outage with the 122 CF/CC & 122 CF/SCOT and must conform to current regulation and standard operating procedures. Training shall cover the topic of system operations and will be conducted at the conclusion of the installation to members of 122 FW/PA, 122 FW/CF, and 122 FW/FSS. Maintenance training will be conducted for the members of 122 FW/CF covering the topics of preventative maintenance and basic maintenance & troubleshooting. Items shall be delivered and installed according to the schedule set forth in the subsequent contract. END PERFORMANCE WORK STATEMENT The following Federal Acquisition Regulation (FAR) provisions [P] and clauses [C] apply to this acquisition: [P] 52.212-1, Instructions to Offerors--Commercial Items. Quotes will be evaluated and awarded to the offeror with the lowest price technically acceptable (LPTA). [P] 52.212-3, Offeror Representations and Certifications -- Commercial Items, and the offeror must include a completed copy of this provision with their proposal. [C] 52.212-4, Contract Terms and Conditions-Commercial Items; [C] 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders--Commercial Items. SITE VISIT: A site visit will be conducted on Friday, 5 Sep 2014, at 10:00 AM EST. We will meet in the Wing Headquarters Conference Room (Building 780, Room 212) and view the job site once all parties have arrived. Information provided at this site visit as well as answers to questions shall not change/qualify the terms and conditions of the solicitation and specifications. Terms remain unchanged unless the solicitation is amended in writing. Please confirm your attendance by contacting MSgt Travis Kraick at travis.kraick@ang.af.mil. You will need to provide the name, DLN (driver's license number), and DOB (date of birth) of all persons in attendance. Government issued ID (e.g. driver's license) will be necessary to gain access to the base.
