Amendment 0001 for W91ZLK-14-T-0637 is issued to provide answers to Questions. The closing date of the solicitation remains the same at September 3, 2014 @ 10:00 AM Eastern Time (EDT). In accordance with the solicitation questions were to be received no later than August 28, 2014 @ 10:00 AM Eastern Time (ET). Questions received after this date may or may not be answered. No telephone requests will be honored. All terms, conditions, requirements and specification of the original solicitation remain the same. Question 1: Would all of the items be shipping together to Aberdeen, or will you need them mailed out to the 19 different facilities directly? Answer 1: All of the spirometers are to be shipped to Aberdeen Proving Ground, Maryland. US Army Public Health Command logistics department will ship to each individual station. Question 2: If you do, do you have a breakdown of how many will be going to each specific station? It would be very helpful in calculating the shipping charge. Question 3: Also will you accept a line item on the quote for shipping, or do you require it to be built into the price of the product? Answer 3: Per the solicitation, the government requires FOB Destination. ------------------------------------------------------------------------------------- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The combined synopsis/ solicitation number is W91ZLK-14-T-0637. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-76. The Government contemplates award of a Firm-Fixed Price (FFP) Purchase Order for Spirometry Equipment and accessories. The equipment being requested are Spirometers, nSpire KoKo PFT Part Number 313105/510000/398200 Brand Name or Equal, in accordance with the attached W91ZLK-14-T-00637 Equipment Specifications and Requirements, Attachment 1 and Diagram Attachment 2. This solicitation is set aside for small business. The Associated North America Industry Classification Code (NAICS) is 339112 and the Business Size Standard is 500. Purchasing of the Spirometer Equipment and Accessories will be in accordance with this solicitation, Attachment 1, and Attachment 2, FAR Part 13 and Subpart 13.5 Test Program for Certain Commercial Items. This requirement is not being procured in accordance with FAR Part 8 and only Open Market, NOT General Service Administration GSA quotations are being accepted from responsible contractors. NOTE to Offerors: Offers shall note the lead time or approximate delivery date for items offered. Acceptance and inspection shall be at Destination. Final completion/destination shall be at US Army Institute for Public Health/Occupational Health Science Portfolio and Public Health Command Regions 5165 Blackhawk Road, Aberdeen Proving Ground, Maryland 21010. Description of the Requirements: The spirometers are to be nSpire KoKo PFT spirometers, Part Number 313105/510000/398200 Brand Name or Equal with accessories in accordance with Attachment 1 W91ZLK-14-T-0637 Specifications and Requirements and Attachment 2 Diagrams, ATTACHED. The award will be made on the basis of the lowest evaluated price of quotations meeting or exceeding the acceptability standards for non-cost factors, which are the technical requirements in accordance with attached W91ZLK-14-T-0637 Specifications and Requirements Attachment 1 and Diagrams Attachment 2. The technical evaluation will be a determination based on information furnished by the vendor. The Government is not responsible for locating or securing any information which is not identified in the quotation. The following factors shall be used to evaluate offers: Lowest Price Technically Acceptable: a. Award will be made on the basis of the lowest evaluated price quotation technically meeting or exceeding the acceptability standards of the required spirometers and accessories in accordance with the Attachment 1 W91ZLK-14-T-0637 Specifications and Requirements and Diagrams Attachment 2 (see the attached). The Government reserves the right to make an award without discussions. Government reserves the right to award on all or none basis. b. Past performance will also be evaluated, the offeror shall propose spirometer equipment that has been manufactured and sold for at least 1 year.
