- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Request for Quote (RFQ) W52P1J-14-T-DB16 is issued as a combined synopsis/solicitation for liquid propane delivery in accordance with the Scope of Work (Attachment 0001), dated 22 August 2014. This combined synopsis/solicitation is prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice and in accordance with simplified acquisition procedures authorized in FAR Part 13. This announcement constitutes the solicitation only; quotes are being requested and a written solicitation will not be issued. This RFQ is issued as a Total Small Business Set-Aside. The Government anticipates issuing a Blanket Purchase Agreement (BPA) for each contractor whose response to this RFQ indicates agreement to the terms and conditions included in Attachment 0002 and are found capable of meeting the Government's requirements for propane delivery, as outlined in Attachment 0001, Statement of Work. The following NAICS code and small business size standard apply: NAICS: 424720 Size Standard: 100 employees The following attachments are hereby incorporated by reference: Attachment 0001 - Scope of Work Attachment 0002 - Contract Clauses/Provisions Attachment 0003 - DFARS Tax Liability Responses shall be submitted via e-mail by 3:00 PM Central Time on 29 August 2014 to the following individual: Mr. David Burke E-Mail: david.burke45.civ@mail.mil Phone: (309)782-7532 As referenced in the addendum to FAR 52.212-1, Instruction - Commercial Items, the following document shall be included with your quote: - A signed copy of Attachment 0003 is required with your submission - A copy of each propane delivery driver's valid commercial driver's license with Hazardous Material (HazMat) endorsement included - Documented proof which shows each propane delivery driver's successful completion of applicable training standards within NFPA 58, CFR 172 and CFR 177, as stated in paragraphs 3.1.1, 3.1.2, 4.1, and 4.2 of the Scope of Work (Attachment 0001) Modification: Extended to 29 September 2014 at 3:00 P.M.
