Description

The Department of Veterans Affairs, VISV 04 Contracting is issuing a combined synopsis/solicitation for a commercial item prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. (ii) This solicitation is issued as a request for quotation (RFQ). Submit written quotes (oral quotes will not be accepted) on RFQ reference number VA244-14-Q-1447. The solicitation is unrestricted. The NAICS code is 325413 and the business size standard is 500 employees. Potential offerors are to quote on the following line items as "brand name." Description of brand name items required: 1.Item #30450-01, Vidas Brahms Procalcitonin (PCT) Test Kits (60/ Kit) "STANDING ORDER - Ship 5 kits per month for delivery on the first Tuesday of every month. Total units: 60 2.Item #30706, Vidas Brahms PCT Quality control Kits (QCV kits - 60/ kit) STANDING ORDER - SHIP 2 kits per quarter in October, January, April and July". Total Units: 8 Period of Performance - October 1, 2014 to September 30, 2015 Contract Type: The government anticipates awarding a Firm Fixed Price award. Awards will be made to the lowest price quote which conforms to the requirements within the solicitation. Delivery: WB VA Medical Center-Warehouse 1111 Eat End Blvd Wilkes Barre, Pa 18711 Quotes are due to this office no later than 3:00 pm, Eastern Standard Time, 12 Sep 14 sent via e-mail to Robert.savage@va.gov. All quotes must include the following information: Company's complete mailing and remittance address, discounts for prompt payment, if any, CAGE Code, Dun & Bradstreet number (DUNS), and Taxpayer ID number. Quotes must be valid for a period of no less than 60 days. Vendors shall indicate, by number of calendar days, the amount of time needed to deliver the items described in the above paragraph. This solicitation documents and incorporates provisions and clauses in effect through FAC 2005-76. It is the responsibility of the contractor to be familiar with the applicable clauses and provisions. The clauses may be accessed in full text at these addresses: https://www.acquisition.gov.
