Bid Schedule. Please compelete and submit with quote. Performance Work Statement This is a combined synopsis and solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 and simplified procedures in FAR Part 13 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This procurement is being issued as 100% small business set-aside. This combined synopsis/solicitation incorporates provisions and clauses in effect through Federal Acquisition Circular 2005-76 released 20140825 and DFARS Change Notice 20140805. The North American Industry Classification System (NAICS) code is 611430 and the size standard is $11M. REQUIREMENT: The Air Force has a requirement for non-personal services for Obstetrics Patient Safety Education Conference Coordinator Services for Wilford Hall Ambulatory Surgical Center at Joint Base San Antonio, Lackland, Texas 78236. Duties include the following: • Organize and manage a Obstetrics Patient Safety Education Conference for San Antonio Military Health System (SAMHS) obstetric physicians, providers, and nurses. • Develop educational conference training based on the direction of Air Force Surgeon General's Consultant for Maternal-Fetal Medicine or designated representative. • Provide 2 - 3 nationally recognized Maternal-Fetal Medicine specialists and 1-2 nationally recognized expert perinatal nurses with experience providing lectures at similar conferences. Coordinate AMA PRA category 1 CMEs for registered attendees. Qualifications: Be a Board Certified Maternal-Fetal Medicine Specialist a sub-specialty of Obstetrics and Gynecology & have a minimum of 10 years of practice in Maternal-Fetal Medicine. Period of Performance: 25 Sep 14 - 19 Dec 14 The government will award a contract on the basis of the lowest priced. Since award will be based on initial responses, offerors are highly encouraged to quote their most advantageous pricing in their initial response. Award will be made using Simplified Acquisition Procedures in accordance with FAR 13.106-2. Your offer must include discount terms, tax identification number, cage code, and DUNS number. To be eligible to receive an award resulting from this RFQ, offerors must be registered in the System for Award Management (SAM) website, no exceptions. To register, please apply via the Internet at http://www.sam.gov. The Government will treat quotes received after the deadline as late in accordance with FAR 52.212-1(f). The Service Contract Act of 1965 does not apply to this requirement because it is for professional services due to the experience and education required to perform the tasks in the Performance Work Statement.
