Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation VA251-14-Q-0658 is issued as a request for quotes (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-12. The North American Industry Classification System (NAICS) number is 811213 and the business size standard is $10 million. This is a small business set aside. All responsible sources may submit a proposal which shall be considered by the contracting officer. Requested Items: Booth Exam Side Expansion (no change to doors) - Qty: 1 Booths 1-3, 36" clear opening exam side doors - Qty: 3 Booth 4, 38" clear opening exam side door (bariatric) - Qty: 1 Installation - Qty. 4 FOB Destination with installation needed by September 30, 2014. Please see attached statement of work for more details. Items will be shipped to the Battle Creek VA Medical Center, 5600 W. Dickman Road, Battle Creek, MI 49307. Offerors must comply with all instructions contained in provision 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition. The following additional provisions and clauses apply: 52.212-2, Evaluation-Commercial Item, the Government will award a purchase order resulting from this solicitation to the responsible offerors whose offer conforming to the solicitation will be most advantageous to the government, price and other factors considered. Price will be evaluated to determine its fairness, completeness, and reasonableness as it relates to the offer. 52.212-3, Offeror Representations and Certifications-Commercial Items, a completed copy of this provision shall be submitted with the offer unless ORCA registered which must be noted in quotation. 52.212-4, Contract Terms and Conditions-Commercial Items. 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, to include clauses: 4,7, 25, 27, 36, 37, 46, 48. The following additional clauses apply: SAM Clause: Attention: You must be registered in SAM to be considered for award. FAR 52.204-7, Central Contractor Registration (Oct 2003) (B)(2) the offeror shall provide its DUNS or, if applicable, it's DUNS+4 number with its offer, which will be used by the contracting officer to verify that the offeror is registered in the SAM database. (C) Lack of registration in the SAM database will make an offeror ineligible for award. Offerors and contractors may obtain information on registration and annual confirmation requirements by calling 1-866-606-8220 or via the Internet at https://www.SAM.gov. Electronic Funds: FAR 52.252-2, Clauses Incorporated by References (Feb 1998). This contract incorporates one or more clauses by reference, with the same force and effect as if they were given in full text. 852.232-72, Electronic Submission of Payment Requests, 852.270-4, Commercial Advertising. DESCRIPTION/SPECIFICATIONS/WORK STATEMENT I.SCOPE OF WORK: The contractor shall provide materials, labor, equipment and supervision for the modification of the audio booths in rooms 1E105, 1E107, 1E109 and 1E11. The sound booth suite consists of a single wall booth on the tester side and a double wall booth on the patient side. The modification will consist of altering four sound booths to enlarge the patient side and reduce the tester side of the booth. The current size of the booth in which the Veteran is placed is too small. The center wall between the patient side and tester side will be moved so that the patient side will gain approximately 2 feet and the tester side will be reduced by 2 feet. Patient side (exam) will change from 5'8" x 6'4" to 5'8" x 8"0'. Audiologist side (control) will change from 7"0' x 6'0" to 7'0" x 4'0". The sound booths are located within the audiology department at the Wyoming Health Care Center. Location for installation will be at 5838 Metro Way Street, Wyoming MI. 49519. II.CONFORMANCE STANDARDS: All services provided under this contract must be performance in conformance with the Federal Regulations and BCVA policies and requirements. III.PREVENTIVE MAINTENANCE: Not Applicable IV.PARTS: To be supplied by vendor. V.REPORTING REQUIREMENTS: The Contractor shall be required to report to Ken Davis VA Senior Engineer, at the clinic site location in Wyoming MI. during normal business hours. This check is mandatory. When the installation is complete, the Contractor shall document services rendered on a legible field service report. The Contractor shall log out with Ken Davis and submit the field report to the Contracting Officer Technical Representative (COR) for a signature, during normal working hours. If the COR is unavailable, a signed authorized copy of the field service report shall be sent to the Contractor after the work has been reviewed. VI.SAFETY REQUIREMENTS: In the performance of this contract, the Contractor shall take such safety precautions as the Contracting Officer may determine to be reasonably necessary to protect the lives and health of the occupants of the building. The Contracting Officer shall notify the Contractor of any safety issues and the action necessary to correct these issues. Such notice, when served on the Contractor or their representative at the work site shall be deemed sufficient for the corrective actions to be taken. If the Contractor fails or refuses to comply promptly, the Contracting Officer may issue an order stopping all or part of the work, and hold the Contractor in default.
