Description
DESCRIPTION - This is a combined synopsis/solicitation issued by the U.S. Fish and Wildlife Service, Region for Nevada Fish and Wildlife Office, 1340 Financial Boulevard, Suite 234, Reno NV 89502-71471 who have a requirement for Veterinary Services for Desert Tortoises for approximately 18 months beginning 10/01/2014. The agency intends to award a Time and Material award with a Not to Exceed amount contract for services to the contractor whose offer conforms to the solicitation and presents the Best Value to the Government. Services include: Health Assessments, Screening for In-State Adoption, Urgent/Emergency Care/Sterilization Program, Euthanasia, Medical Records Maintenance. Qualifications include: Contractor must be a State certified licensed veterinarian and must be within 50 miles of the Las Vegas, NV metro area. Contractors must be registered in the System for Award Management (SAM) database www.sam.gov. This announcement constitutes the only solicitation whereby a formal solicitation will not be issued: The solicitation number F14PS01279 is issued as a request for quotations. This procurement action is 100% small business set-aside. The NAICS code is 541940, small business size standard 7.5M. This acquisition is set aside for small business but quotes from all businesses will be considered in the event there are no responsive small businesses. BASIS OF AWARD: The government anticipates award or a contract resulting from this solicitation to the responsive/responsible offeror whose quote conforms to the solicitation and is most price advantageous to the government. Award will be made on a best value analysis to include price, past performance, certifications, and quality. Price is equal or more important than other factors? FAR 52.252-2 Clauses Incorporated by Reference This contract incorporates one or more clauses by reference, with the same force and effect as if they were given in full text. Upon request, the Contracting Officer shall make their full text available. FAR 52.212-1, Instructions to Offerors - Commercial Items; FAR 52.212-2, Evaluation - Commercial Items; FAR 52.212-3, Offeror Representations and Certifications - Commercial Items, must be completed on-line at https://www.sam.gov. FAR 52.212-4, Contract Terms and Conditions - Commercial Items; FAR 52.212-5, and 52.232-33 Payment by Electronic Funds Transfer-IPP. Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items, SUBMISSION REQUIREMENTS: OFFERS ARE DUE Not later than September 12, 2014 AT 12:00 Noon PST. Offers may be submitted on letterhead and must include, price list for services pertaining to this scope, prompt payment terms, contract remittance address, DUNS number, federal tax identification number, name, phone number and address of your point of contact, a completed copy of FAR 52.212-3, and the proposed submittals described above. Offerors should include experience similar in complexity to the scope with their offer. Also include any proof of certifications/licensing as requested above.
