This is a Combined Synopsis/Solicitation for commercial items prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only Solicitation; quotes are being requested and a written Solicitation will not be issued. Solicitation number VA249-14-Q-0893 is issued as a (Request for Quotation (RFQ). This requirement is unrestricted. NAICS: The NAICS is 332913. To be receiving a government award, the offeror must be currently registered in System for Award Management (SAM) at www.sam.gov. Offerors are encouraged to go to the SAM database www.sam.gov and start the registration process as soon as possible to avoid possible delays should the contractor be selected as a prospective awardee. Online Representation and Certification Application (ORCA) must also be current to qualify to receive a contract award. Offeror may register with CCR at www.sam.gov Description of Supplies/Services: The Network Contracting Office 9 (NCO9) has a requirement for a plumbing protection system needed at the Cooper & Leestown Division of the Lexington VA Medical Centers located in Lexington, KY. This requirement is for the installation and training of staff to prevent paper towels, barrier wipes, and other products from being flushed down toilets at each location. The system will be installed by the contractor and the Traptex retrievers will be handed over to the Medical Center staff along with training on how to use the retrievers. The contractor will be responsible for bringing a technician to custom fit the system into existing toilets. Vendors are asked to submit quotes for products that are Brand name or Equal to the product requested. LI 002: TRAPTEX PLUMBING PROTECTION SYSTEM ALONG WITH INSTALLATION AND TRAINING AT THE LEXINGTON VA MEDICAL CENTER LEESTOWN DIVISION 2250 LEESTOWN RD LEXINGTON, KY 40511 55 EA. LI 003: TRAPTEX WIPE RETRIEVERS 6 BX. Contract Type: The government anticipates awarding a Firm Fixed Price award. Award will be made to lowest price quote which conforms to the requirements within this solicitation. To receive consideration as SDVOSB or VOSB, the firm must be registered and verified in the VIP database at www.vip.vetbiz.gov. Submitting Quote: Contractor shall submit their quote on company letterhead and shall include unit price, total, unit qty and item description as specified above, as offered discounts, proposed delivery time, name, address, and telephone number of the offeror, firm's DUNS# and ORCA document in SAM at www.sam.gov, terms of any express warranty, unit price, overall total price, applicable shipping charges, completed copy of 52.212-3 Offerors Representations and Certifications- Commercial Items, and ORCA document. Quotes must be received September 13, 2014 by 12pm CST. All quotes must be submitted by electronic submission in reference to VA249-14-Q-0893.
