- 14Q0054 Provisions and Clauses 14Q0054 Required Provisions 14Q0054 Required CLIN Structure 14Q0054 Request for Quote (RFQ Amendment #1) * See the attachments for the complete detail The purpose of this Amendment #1 is to change the GSA competition language and to clarify our requirement [see sections highlighted in yellow]. Offerors must include a statement in their offer that they acknowledge this RFQ Amendment #1. All other terms and conditions remain unchanged. General Overview: The Uniformed Services University of the Health Sciences (USU), located in Bethesda, MD, has a requirement for a replacement uninterruptible power supplies (UPS) system. The USU is the only Federal health sciences university and we are committed to excellence in military medicine and public health during peace and war. We provide the Nation with health professionals dedicated to career service in the Department of Defense (DoD) and the United States Public Health Service (PHS) and with scientists who serve the common good. We serve the uniformed services and the Nation as an outstanding academic health sciences center with a worldwide perspective for education, research, service, and consultation; we are unique in relating these activities to military medicine, disaster medicine, and military medical readiness. USU is located in Bethesda, MD on the grounds of the Naval Support Activity Bethesda. Additional information regarding USU and its mission can be found on our website: www.usuhs.mil. This is a combined synopsis/solicitation for commercial items in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This is a Request for Quotes (RFQ) for commercial items in accordance with the procedures of FAR Part 12 Acquisition of Commercial Items and Part 13 Simplified Acquisition Procedures. This RFQ incorporates all provisions and clauses in effect through Federal Acquisition Circular FAC 2005-76, effective August 25, 2014 and Defense Federal Acquisition Regulation Supplement (DFARS), effective August 28, 2014. Offerors are encouraged to present their best price, including but not limited to their GSA price. This requirement is set-aside for small business concerns under North American Industry Classification System (NAICS) Code 335911 Storage Battery Manufacturing with a size standard of 500 employees. This NAICS Code meets the Non-Manufacturer Rule Waiver. Small business resellers may offer other-than-small manufacturer's products. In order to be eligible for award, offerors must be registered on SAM.gov with completed Representations & Certifications (Reps & Certs). Offerors not registered on SAM.gov with completed Reps & Certs will not be eligible for award. Offerors interested in responding to this requirement must provide a quote for a UPS system, which at a minimum, meets the specifications listed below. A single firm fixed-priced (FFP) purchase order will be awarded. Minimum Specifications: USU intends to replace the existing UPS system, which supports and powers the data center. We have determined that the APC InfraStrucXure Symmetra PX UPS 60kVA/kW system that is expandable up to 100kVA/100kW will fully meet our needs; however, offerors are allowed to present comparable replacement brands and models as long as they meet the following minimum specs: a.Input: 480 VAC 4 wire + ground, 60 Hz; b.Output: 208/120 VAC 4 wire + ground, 60 Hz; c.Battery Back-Up: 12 minutes for Full Load and 18 minutes for Current Load; d.Software that allows the monitoring the state of UPS via network; e.Removal of existing UPS system and installation of the new UPS system; f.Replacement of wall-mounted transformer that can support up to 100kVA/100kW; g.One-year warranty (plus four option years of maintenance + 2 annual preventative maintenance (PM) visits). USU currently has an Emerson Accupower AP526 UPS System with 90 Power PC-12120S batteries. The transformer is Acme Electric Transformer T-1-53316-3S. USU may decide to keep these units. In such case, the awardee will be responsible for removing these items and move them to the USU Facilities-located on the same campus. We require up to five years of warranty/maintenance coverage for the new system. We require at least one year of manufacturer's warranty up to five years. If a five-year warranty is not available, we require vendors to offer a separate annual maintenance and 2 annual PM visits for the new system for up to five years (including the warranty), i.e. if the system comes with 1-year warranty, offeror must present 4 additional years of annual maintenance and PMs; if it comes with 2-year warranty, offeror must present 3 additional years of annual maintenance and PMs; etc. Please see the CLIN schedule table below and the attached CLIN Structure spreadsheet. Awardee must be available Monday through Friday 7:00A to 4:00P and all issues must be addressed within 72 hours upon our service call and resolved as soon as possible before creating any productivity-related delays. The period of performance (PoP) for the maintenance will begin upon installation of all the required units and parts. We will adjust the PoP accordingly for the Options. We are unable to supply the complete detail, e.g. the existing system, available dimensions, etc., for various reasons. USU plans to hold a site visit on September 10, 2014 [see section Site Visit]. Although this is not a required site visit, because we're unable to extract all the information, interested vendors are highly encouraged to attend. Site Visit: One scheduled site visit will be held on September 10, 2014 at 2:00PM at the USU, Building A, Room 1040C. All interested offerors are highly encouraged to attend to survey the space. The site visit is subject to FAR subpart 52.237-1 Site Visit. Attendees should allow adequate time to gain access onto the Government facility. Due to heightened security measures, everyone entering the base must have a Federal-Government-issued ID card or a military ID. Otherwise, the offeror must be placed on the access list; those who request to be placed on the access list must provide (1) names of the attendees, (2) company name, (3) a phone number, and (4) the vehicle make, model, and color in which you'll be arriving by 12:00P EST (noon) on September 10, 2014. Evaluation / Award Detail: The Government intends to award a single firm-fixed-priced (FFP) purchase order to the offeror representing the best value to the Government based on the evaluation factors listed below. Evaluation factors are listed in order of importance, with most important listed first. The Government will conduct a trade-off between price and non-price factors. The Government is not required to award to the lowest priced offeror or the offeror quoting the highest technically rated item. Evaluation Factors (in order of importance): 1.Technical. Offerors must provide product literature, which demonstrates the capabilities of the quoted item(s). At a minimum the product literature must demonstrate that the quoted item meets the specifications outlined above. Information on warranty, training, installation, and delivery lead-time must be included in the quote submission and will be evaluated. The Government reserves the right to utilize technical (e.g. trade magazines) and customer reviews/references as part of the technical evaluation. 2.Corporate Experience. Offerors are required to provide a narrative of their corporate experience in removing and installing the required units. 3.Price. Offerors must include all applicable costs (e.g. components, removal, installation, and shipping) in their quote. The total quoted price for all components will be the evaluated price. 4.Past Performance. The Government may evaluate the offeror's present and past performance on similar tasks within the past three years. Offerors must provide two (2) customer references for similar work performed. The Government reserves the right to select which reference(s) to contact. The following information must be provided for each reference: a.Name of client and address; b.Name of current PoC, including title, phone number, and email address; c.Contract number, if available; d.Contract value (total cost); e.Contract period of performance (PoP); and f.A clear description of the service provided. Vendors must indicate whether they accept credit card payments and whether this would incur additional fees. USU cannot make a payment via third-party-payment merchants, e.g. PayPal. Quoted items must be new. Quotes for refurbished equipment will not be considered. Offerors must also indicate in quote where item is manufactured. Offerors may provide quotes for multiple UPS systems. Items will be shipped to Bethesda, MD. See attached list for applicable provisions and clauses for this requirement. In the preparation of the quote, please be advised that USU requires the quote to be valid until October 31, 2014. The following clause applies to this contract: 52.232-18 Availability of Funds The Government reserves the right to award without discussions. Questions/clarifications regarding this solicitation must be submitted via email to andrew.lee@usuhs.edu by 2:00P EST on September 8, 2014. Any question received after 2:00P EST on September 8, 2014 will not be answered. Questions will not be answered over the phone or submitted by any other means. However, those vendors that attend the site visit on September 10, 2014 will be allowed to ask questions directly from the start to end of the site visit. All questions will be answered in an amendment to the solicitation posted on or around September 11, 2014. Quote packages are due by 10:00PM EST on September 12, 2014.
