DFARS 252.209-7993 Atch 4 Background Check Release Form Atch 3 Wage Determination Atch 2 Halon Statement of Work Atch 1 Halon Fire Extinguisher Pricing Schedule This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. Solicitation number "F2R3C54191A003, Halon Fire Extinguisher Maintenance" is hereby issued as a Request for Quotation (RFQ) using FAR Subpart 13.1 Simplified Acquisition Procedures. This requirement is 100% Small business Set-Aside and is under North American Industry Classification System (NAICS) code 811310 with a small business size standard of $7.5 million. Please identify your small business status in your response based upon this standard. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-76, effective 25 July 2014 and the Defense Federal Acquisition Regulation Supplement Change Notice DPN 20140805 (Effective 05 Aug 2014), Air Force Acquisition Circular (AFAC) 2014-0421, effective 21 April 2014. Please ensure quote is valid until 1 October 2014. SERVICES: Halon Fire Extinguisher Maintenance Provide all labor, transportation, equipment, and supplies for Halon Fire Extinguisher Maintenance, see attached Pricing Schedule (Atch 1) for extended line item description, and the Statement of Work (Atch 2) for service summary and details. It is the responsibility of the offeror to be familiar with the applicable clauses and provisions. Clauses and provisions may be accessed via the Internet at website http://farsite.hill.af.mil. The provision FAR 52.212-1 Instructions to Offerors - Commercial Items (Apr 2014) and the applicable addendum apply to this acquisition. The addendum references FAR 52.212-1(b)(8) and is hereby tailored as follows: FAR 52.212-1 Addendum SUBMISSION OF COMPLETE QUOTES: All quotes must be complete, self-sufficient, and respond directly to the requirements of this solicitation. Failure to furnish a complete quote, at the time of quote submission, may result in the quote being unacceptable to the Government and eliminated from consideration for award. Corrosion Control Plan Atch 1, Pricing Schedule Offerors shall include with their offer a completed copy of the provision FAR 52.212-3 Offeror Representations and Certifications-Commercial Items (Nov 2013) and Alternate I (Apr 2011). An offeror shall complete only paragraph (b) of this provision if the offeror has completed the annual representations and certifications electronically at http://www.acquisition.gov. If an offeror has not completed the annual representations and certifications electronically at the System for Award Management (SAM) website, the offeror shall complete only paragraphs (c) through (o) of this provision. In the event the Government requires continued performance, the Government's evaluation of prices proposed will be considered fair and reasonable as applied to an extension of services. Should an extension of services be required rates will be adjusted only as result of revisions to applicable prevailing Department of Labor wage rates. No award will be made under this solicitation until funds are available. The Government reserves the right to cancel this solicitation, either before or after the closing date. In the event the Government cancels this solicitation, the Government has no obligation to reimburse an offeror for any costs." Addendum, Smoking in AETC Facilities Contractors are advised that the Commander has placed restrictions on the smoking of tobacco products in AETC facilities. AFI 10-42, Tobacco Use in the Air Force, and its AETC Supplement 1, outline the procedures used by the Commander to control smoking in our facilities. Contractor employees are subject to the same restrictions as government personnel. Smoking is permitted only in designated smoking areas. This purchase will incorporate one or more clauses by reference, with the same force and effect as if they were given in full text. A Firm-Fixed Price purchase order will be issued in writing to the successful offeror. To be eligible to receive an award resulting from this solicitation, contractor must be registered in the System for Award Management (SAM) www.sam.gov. Use Atch 1 as the quote format. Responses are to be added into the format under PRICE, UNIT, and AMOUNT sections provided in Atch 1. Email responses are preferred. No telephonic responses will be processed.
