- This is a Combined Synopsis/Solicitation (CSS) for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included within this notice. This announcement constitutes the only solicitation. Quotes are being requested and a written solicitation will not be issued. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-76, Effective 25 Jul 2014 and Defense Federal Regulation Supplement (DFARS), current to DPN 20140731 (Effective 31 Jul 2014) Edition. It is the responsibility of the contractor to be familiar with the applicable provisions and clauses which can be accessed in full text at www.farsite.hill.af.mil. This solicitation is being issued as a Request for Quotations (RFQ) under Solicitation Number W912LA-14-T-6003, which will result in a single-award, firm fixed price contract. This requirement is for Brand Name of Equal, Carestream Health, Inc phosphor imaging plates to include rigid cassettes. The manufacturer is Carestream Health, Inc. This equipment shall be compatible with the existing Virtual Media Integrations 5100MS scanning unit and computer (stock number 6635015731461). This is an addition to the existing make and model imaging plates. This action is being solicited as a one hundred percent small business set aside. The North American Industry Classification System (NAICS) code is 334517 Irradiation Apparatus Manufacturing with a size standard of 500 employees. CLIN 0001, 14 x 17 HR Imaging Plate FFP 14 x 17 HR Imaging Plate, Carestream (PL012 or 8360521), Must be compatible with VMI 5100MS Premium CR Digitizer. Quantity: 50 each CLIN 0002, 14 x 17 Vinyl Cassette FFP 14 x 17 Vinyl Cassette, Must be compatible with VMI 5100MS Premium CR Digitizer. Quantity: 50 each The requirement will be solicited and evaluated under FAR Part 13.5, Simplified Acquisition Procedures. Contract award shall be made to the offeror whose offer will result in the best value to the Government. Offerors must comply with all instructions contained in FAR 52.212-1, Instructions to Offerors - Commercial Items, which applies to this acquisition. The following addenda apply to this solicitation. Offers will be evaluated on price and technical acceptability. The Government shall not be responsible for locating or obtaining any information not identified in the offer. If an offer cannot comply with every requirement, that offer shall be deemed to be technically unacceptable, and therefore will not be considered. Once an offer has been determined to be technically acceptable and in compliance with all directions in this solicitation, the Government will award to the offeror with the lowest evaluated price.
