Amended solicitation 2 I. GENERAL INFORMATION The 42nd Contracting Squadron, 50 LeMay Plaza, Bldg 804, Maxwell AFB, AL has a requirement to purchase services as described in this document. This is a streamlined combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only notice. A written solicitation will not be issued. This requirement is issued as a request for proposal for F2X0CH4199A001- Catholic Music Director. The solicitation document and incorporated provisions and clauses are those in effect the Federal Acquisition Circular 2005-74 (01 Jul 14) and the DPN 20140721 effective 21 July 14. A Firm Fixed Priced Award will be made in accordance with the following: NAICS Code: 711130, size standard $7 Million Notice to Offeror(s): THE GOVERNMENT IS SOLICITING FOR "OPEN MARKET" PROPOSALS ONLY. GENERAL SERVICES ADMINISTRATION (GSA) PROPOSALS WILL NOT BE CONSIDERED. THIS REQUIREMENT IS SUBJECT TO THE AVALIBILITY OF FUNDS. II. NATURE OF ACQUISITION: Maxwell AFB has a requirement to purchase the below listed services. This procurement will be awarded under Simplified Acquisition Procedures. All inquiries/responses to this notice must be received by 15 September 2014, 1:00 P.M CST. III. SCHEDULE OF SUPPLIES/SERVICES: The contractor shall provide the NON-PERSONAL service listed below, which is to be performed on Maxwell, AFB in accordance with the SOW. This is a small business set aside requirement for F2X0CH4199A001. Firm fixed pricing is being sought. This is being solicited in accordance with FAR 13. IV. PROPOSAL PREPARATION INSTRUCTIONS To ensure timely and equitable evaluation of quotes, offerors must comply with instructions contained herein. Quotes must be complete, self-sufficient, and respond directly to the requirements of this RFP. 1. The offeror must submit a signed and dated quote on company letterhead with unit prices and extended prices in response to this solicitation. In doing so and without taking any exception to the requirements of this synopsis/solicitation, including specification, "Schedule of Supplies/Services" and terms and conditions, the offeror agrees to accede to all solicitation requirements. Offerors must clearly identify any exception to the solicitation terms and conditions and provide complete accompanying rationale. 2. If offerors ORCA Representations and Certifications are not current, then they must include a completed copy of the provisions at FAR 52.212-3, and Alternate I, Offeror Representations and Certifications - Commercial Items. Incomplete or expired Representations and Certfications will render a proposal as non-responsive and it will not be considered. 3. Interested parties capable of providing the requirement must submit a written proposal to include discount terms, tax identification number, cage code, and DUNS. 4. To be eligible to receive an award resulting from this RFP, contractors must be registered in the DoD Systems for Award Management database, no exceptions. The offeror by acceding to all of the requirements of this synopsis/solicitation will be determined technically acceptable. Award will then be based on lowest price that was technically acceptable. Awardee will have to do complete a proficiency demonstration before services begin. VI. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extention of performance hereunder shall not exceed 6 months.
