Description
Sources Sought Notice The PVAMC provides a full range of cardiac monitoring services to patients within the Portland VAMC, Roseburg VAMC and White City VAMC. Cardiac symptoms are often transient in nature such that it is difficult to obtain an EKG the exact time they occur. Cardiac monitoring if used for patients experiencing symptoms, such as palpitations, syncope, dizziness and shortness of breath. Patients are provided a state of the art sensor and cell phone and are monitored according to device type and diagnostic information needed. The varieties of devices have the ability to auto-trigger, auto-send, manually trigger and/or manually send depending on the device ordered to evaluate patient's symptoms. The PVAMC will establish customized physician notification criteria for office hour, twenty-four hour and emergency criteria in order to effectively triage data and ensure appropriate patient care. The Contractor will post all reports to their website for retrieval and download to Vista Imaging/CPRS. The main point of contact for outpatient cardiac monitoring is located at PVAMC who, in turn, coordinates care with Roseburg VA and White City VA to ensure quality care and follow-up regarding cardiac monitoring. The Contractor shall be responsible for providing the full spectrum of outpatient cardiac monitoring services to all beneficiaries serviced by the PVAMC. The contractor shall provide all supplies, materials, equipment, mail out service of equipment, equipment maintenance and patient education as necessary for patients receiving cardiac monitoring as specified under this Statement of Work (SOW). These services shall be required twenty-four hours a day, seven days a week, including holidays. The contractor shall be staffed and stocked to render services provided in accordance with all terms and conditions of this SOW for the duration of the performance period. Vendors should thoroughly review the specifications and be familiar with the area of coverage before submitting their proposal in order to be fully aware of the supplies and services required. Lack of understanding will not relieve the vendor from performing in accordance with the strict intent and meaning of the specifications and will not authorize additional cost to the Government. Firms offering a response to this notice should ensure that their registration in the System for Award Management (SAM) database https://www.sam.gov/portal/public/SAM/ is current and should ensure that the applicable NAICS code 334510 is included in the SAM profile prior to submission of quotations. The anticipated Base period of this contract shall be from 11/01/2014, through 09/30/2015. There shall be 2 possible option years to exercise, depending on the needs of the VA. The solicitation will result in the award of a single contract. Please see the service schedule and statement of work below for service requirements.
