Description
The Bureau of Indian Affairs (BIA) is soliciting offers to procure for construction supplies for Division of Transportation, Force Accounts Farmington, NM. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Solicitation No. A14PS01082, herein cited is issued as a Request for Quote (RFQ) and will be procured under FAR Part 12, Acquisition of Commercial Items. Solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-73. This acquisition is for a Native American Owned Business Small Business Set-Aside under the Buy Indian Act (25 U.S.C). The NAICS Code is 212321, Size standards 500.. Scope of Work: Contractor shall provide all labor, equipment, materials, and incidentals required to deliver the following materials Red Valley, AZ project site: Hot Asphalt Concrete Pavement, shall conform to Section 403.00 of the standard specification, FP-03,an d subsections 403.2, and 403.3 mix design specification. Job mix formula shall meet 3/4" class B, SP3 and SP4 applicable material, FP-03, Section 702 & Section 703 requirements and appropriate design parameters. Furnish a commercial certification identifying the batch and/or lot number, material, quantity of batch, date and time of production, and the name and address of the production. FOB delivered to Red Valley Day School, Red Valley, AZ. - Project site, includes delivery charges and other charges. Vendor shall be within 150 miles of Red Valley, AZ project site. Estimated delivery and sales tax = _____. Total = _____. CONTRACT CLAUSES: The following clauses apply to this acquisition: FAR 52.212-1, Instructions to Offerors Commercial Items; FAR 52.212-3, Offeror Representations and Certifications Commercial Items; FAR 52.212-4, Contract Terms and Conditions Commercial Items; and FAR 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders Commercial Items. In paragraph (b) of FAR 52.212-5(b), the Contractor shall comply with the following FAR clauses that the Contracting Officer has indicated as being incorporated in this contract by reference to implement provisions of law or Executive order applicable to acquisitions of commercial items: FAR 52.223-18, Encouraging Contractor Policies to Ban Text Messaging while Driving (AUG 2011); FAR 52.225-1 Buy American Act-Supplies (June 2003) (41 U.S.C. 10a-10d) and FAR 52.232-18, Availability of Funds (April 1984).
