Description
Attachment 1 The US Marshals Service (USMS) is issuing a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. Solicitation DJM-14-A41-Q-8080 is being issued as a Request for Quotation (RFQ). USMS will award a firm fixed price type contract. This requirement is a 100% small business set-aside. USMS is accepting bids for the purchase of 2,000 individual trauma kits. The specific items that should be included in the individual trauma kits can be seen in Attachment #1, along with the required quantity for the particular item. The price quote shall include all delivery costs. The provisions and clauses incorporated into this solicitation document can be seen below. 1) 52.247-34, FOB Destination 2) 52.212-1, Instructions to Offerors - Commercial 3) 52.212-2, Evaluation - Commercial Items. The following text is added to Paragraph (a) of FAR 52.212-2: (Low Price Technically Acceptable). A competitive award will be made to the responsible firm with lowest priced technically acceptable quote. 4) 52.212-3, Offeror Representations and Certifications - Commercial Items 5) 52.212-4, Contract Terms and Conditions - Commercial Items 6) 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders - Commercial Items. The following FAR clauses identified at paragraph b of FAR 52.212-5 are considered checked and is applicable to this acquisition; o 52.203-13, Contractor Code of Business Ethics and Conduct o 52.209-6, Protecting the Government's Interest When Subcontracting with Contractors Debarred, Suspended, or Proposed for Debarment. o 52.219-6, Notice of Total Small Business Set-Aside o 52.222-19, Child Labor--Cooperation with Authorities and Remedies o 52.222-21, Prohibition of Segregated Facilities o 52.222-26, Equal Opportunity o 52.222-35, Equal Opportunity for Veterans o 52.222-36, Affirmative Action for Workers with Disabilities o 52.225-13, Restrictions on Certain Foreign Purchases o 52.232-33, Payment by Electronic Funds Transfer--System for Award Management 7) USMS-0001, Release of Residual Funds 8) USMS-0002, Release of Residual Funds 9) 52.211-5, Material Requirements All offerors MUST be registered in the System for Award Management. An award will be made to the lowest priced bidder who can conform to the technical requirements listed in Attachment #1. All items MUST be new. USMS will not accept any used products. Delivery/Shipping Location: USMS/Special Operations Group 1401 F Street Camp Beauregard Pineville, LA 71360 Note: The Contractor shall deliver all items FOB Destination to the address identified above. Due to the large volume of individual trauma kits solicited, the contractor that is awarded the contract SHALL ONLY deliver 10 individual trauma kits to the address below within 14 days of contract award for inspection. USMS personnel will review the kits for acceptability and make sure the individual trauma kits are prepared in accordance with the specifications outlined in Attachment #1. If the 10 individual trauma kits are deemed acceptable by USMS, USMS personnel will request that the contractor deliver the remaining 1,990 individual trauma kits within 3 business days of receiving the 10 kits. The contractor SHALL deliver the remaining 1,990 individual trauma kits within 90 days after receiving concurrence from USMS personnel. To be eligible for this award, offerors must include the following within their overall proposal: The North American Industry Classification System code is 423450 and a single award will be made. All quotes are due on September 18, 2014 at 7:00am (EST) and must be submitted electronically to Broderick Morris at Broderick.Morris@usdoj.gov. No information or questions requesting clarification will be provided over the telephone from prospective offerors. Late quotations will not be considered for award. Interested offerors must submit in writing any questions concerning this solicitation no later than 1:00pm (EST) on September 15, 2014. Please submit all questions to Broderick Morris at Broderick.Morris@usdoj.gov. All quotes shall be submitted on Company Letterhead and include your firm's proposed pricing, Cage Code, Tax ID Number, and DUNS Number. Late quotations will not be considered for award.
