Description

Provide Local Dental Laboratory Services in accordance with the SOW. This commercial dental laboratory is required to be within 30 miles from VAPHS Heinz Campus. The Contractor shall fabricate all prostheses and perform all laboratory steps to the highest standard of quality, with special attention given to neatness and laboratory prescription detail. Materials used shall be consistent with those listed in the schedule and should meet all Food and Drug Administration (FDA) requirements and approval. The Contractor's laboratory facility must be certified by the American Dental Association (ADA), National Association of Dental Laboratories (NADL), or equivalent. The Contractor shall provide a copy of all relevant licenses, permits, registrations and certifications inclusive of any sanctions current or pending throughout the United States of America along with proposal.
