Description
The VA Pittsburgh Healthcare System (VAPHS), intends to negotiate on a Sole Source basis with Sapience Technologies, 95 Club Court, Alpharetta, GA 30005-8774, to provide services to upgrade VAPHS existing Sapience Technologies Critical Care Genie Software, in accordance with the following Scope of Work and Security Requirements: Estimated Contract Amount: $110,000 Scope of Work: Targeted Major Benefits Improve GENIE usability and capability by: (i) distinguishing treatment intent; (ii) extending instructions (for euglycemic termination); (iii) enabling RN's to treat with a less aggressive Intensive Insulin Therapy (IIT); (iv) enabling a D5 'safety net' during rapid hyperglycemia reduction; (v) extending algorithm to manage effects of pressor administration, extubation, Amiodarone and Vancomycin; (vi) extending algorithm to cover patients with Type 1, DKA and HONK; and (vii) enabling immediate access to Policies on ICU Glycemic Management. Needs Identified A.Need to Upgrade Critical Care GENIE based on current deficiencies as follows: a.Create a new Interface that provides separate Options for i.Choice 1: Initiate GENIE for starting insulin drip ii.Choice 2: Maintain GENIE for ongoing insulin drip iii.Choice 3: Restart GENIE after stopping drip iv.Choice 4: Discontinue GENIE for euglycemia off insulin drip b.Create a second screen for Choice 2, Maintain GENIE, to differentiate between: i.Patients with pre-existing diabetes (more aggressive in target range) ii.Patients with stress hyperglycemia and no preexisting DM (less aggressive in target range) c.Reintroduce 5% Dextrose "safety net" d.Tighten GENIE responses in 140-150mg/dl BG range to accurately reflect target of 90-140mg/dl, rather than the nominal 90-140 range currently, with no response until BG >150. e.Introduce Options for modifying GENIE responses based on the following: i.Stopping/starting pressors (include number of pressor drips?) ii.Extubation iii.Amiodarone and Vancomycin drips (in D5w) f.Include radio button on the first screen for nurses to "Review Policies on Glycemic Management in ICU" as needed (will need to incorporate this text in the GENIE program) B.Need to Create three new versions of GENIE to satisfy unfilled needs, as follows: a.Type 1 GENIE b.DKA GENIE c.HONK GENIE Programming time required for the upgrade/redesign: Grand Total: 1100 hours Justification (Breakdown of Specific Tasks that must be completed to accomplish the above) 1.Migrate GENIE from EXCEL to ACCESS: The current MS-Excel platform for GENIE has reached its limits in terms of the ability to include the above. Thus, the redesign can only be accomplished by migrating GENIE to MS-ACCESS (the programming language remains unchanged but the program will have to be rewritten to accommodate the above with the greater flexibility of ACCESS and the options available in it (200 hours of programming to migrate to ACCESS) 2.New Interface with 4 new options/choices. This means that the "entry point" into the GENIE logic will be different for each option, which in turn requires the programming logic to be rewritten to accommodate these different entry points into the core program (100 hours) 3.Creating 2 versions for nurses to implement either a less or more aggressive approach when BG is in the target range will require the core program logic to be rewritten to allow this (100 hours). 4.Adding the D5W safety net to prevent hypoglycemic excursion below 70mg/dl: This will require new logic to accommodate a varying D5W drip when BG is <120mg/dl, create a new paradigm for adjusting insulin drip rates in the context of a slower decline in BG (100 hours) 5.Tighten responses in 140-150mg/dl range: New logic to be added (15 hours) 6.Introduce Options for modifying GENIE responses for pressors, etc.: New logic (30 hours) 7.Incorporate GENIE manual/ICU Policies relating to GENIE: (5 hours) 8.Creating 3 New versions: The new versions are unrelated to each other or to Critical Care GENIE, so logic needs to be derived and programmed from scratch): 150 hours to program and design an interface for each version (450 hours) 9.Beta-testing final versions and making adjustments/corrections (100 hours): a.Upgraded Critical Care GENIE (25 hours) b.Type 1 GENIE (25 hours) c.DKA GENIE (25 hours) d.HONK GENIE (25 hours) Estimated Timeline: Per items listed in "Justification (Tasks)", above, Sapience Technologies will deliver the following: Phase I: Items 1 and 2 within 8 weeks of project initiation Phase 2: Item 3 by week 10. Phase 3: Items 4, 5, 6 and 7 by week 14. Phase 4: Item 8 by week 24. Phase 5: Item 9 (Depends on clinical use and testing). Anticipate wrap up by week 28. Security Requirements: Contractor will be required to comply with physical security guidelines by either checking in with the VA Police each time they come on-site to perform contracted services or by obtaining a VA Contractor ID badge from the VA Police. VA ID badge must be worn at all times while on VA premises. Contractor owned computer equipment including laptops are not permitted to be connected to the VA network. Any removable storage device used in medical equipment must be scanned by Biomed or OIT. The following language from VA Handbook 6500.6 is required in this contract: o Appendix C: Paragraphs #3a, #4a,b,c,d,e,g,I,j,k,l, #5h, #6a,b, #7a, #7d, #9(all) Contractor must complete "VA Privacy and Information Security Awareness and Rules of Behavior" training prior to the performance of the contract and annually thereafter. Training must be completed in VA's TMS system (https://www.tms.va.gov/). Contractors must use the TMS Managed Self Enrollment method to complete the training in TMS. The COR must ensure that all contractors are validated in the PIH domain. Proof of training completion must be verified and tracked by the COR. This Notice of Intent is not a request for competitive proposals. Parties may identify their interest; accompanied by supporting literature, to the Contracting Officer no later than 4:00PM on Monday 09/15/2014. Responses should be sent to ronald.mazzei@va.gov. The determination to conduct a competitive procurement based on responses to this notice is solely within the discretion of the Contracting Officer.
