Dyess Form 173 for Base Access Site Visit Registration Form Wage Determination Scope of Work 7TH CONTRACTING SQUADRON (ACC) 381 Third Street Dyess AFB, Texas, United States 79607 PREPARED BY: A1C JonNeisha McNabb Combined Synopsis/Solicitation Install Three Warm Air Handler Units - Reference # F1R31A4246A004 SOLICITATION DATE: 12 September 2014 QUOTES DUE: 22 September 2014, NLT 1400 CST Requesting Agency: 7th Civil Engineering Squadron - 7 CES/CEAN Dyess Air Force Base, Texas, United States Purchasing Agency: 7th Contracting Squadron - 7 CONS/LGCA Dyess Air Force Base, Texas, United States 79607 This is a combined synopsis/solicitation for a commercial item prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation is issued as a request for quotation IAW FAR Part 12 and 13. Submit quotes in reference to Request for Quote (RFQ) reference number F1R31A4246A004. This requirement is 100% Set-Aside for Women Owned Small Business. The NAICS code for this requirement is 238220, "Plumbing, Heating, and Air-Conditioning Contractors", with a small business size standard of $14 Million. Notice to Offeror (s)/Supplier (s): Funds are not presently available for this effort. No award will be made under this solicitation until funds are available. The Government reserves the right to cancel this solicitation, either before or after the closing date for receipt of proposals. In the event the Government cancels this solicitation, the Government has no obligation to reimburse an offeror for any costs. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-76 effective 25 Aug 2014 and Defense Federal Acquisition Regulation DPN 20140828 effective 28 Aug 2014 and Air Force Federal Acquisition Regulation AFAC 2014-0421 effective 21 Apr 2014. Quote shall include a unit price and total price for the following bid schedule: BID SCHEDULE Line Item Number Description Period of Performance Quantity Unit Unit Price Total Price 0001 Replacement of Three Warm Air Handlers 15 Business Days to Complete Work; Work shall begin within 5 days of Award. 1 LO • The contractor shall have 15 days to complete the replacement of the Air Handler Units IAW Statement of Work. • Prices shall be all inclusive (i.e. all material, transportation, labor, etc.). • Shipping will be F.O.B Destination • Prices shall be quoted in USD ($) and quotes shall be valid for a minimum of 60 days. Pre-Bid Site Visit: DATE: Site Visit will be held on 17 September 2014 @ 01:00 PM (CST) at Dyess AFB TX. All parties will meet at the address below before going to the site. Quotes must be valid for a period of no less than 60 days. Price Information: Vendor must complete the bid schedule found on page 2 of this solicitation. Evaluation of Quotes: The Government's evaluation of quotations will fully account for the evaluation factors identified and required under the quote specifics above. Quotes that do not meet the quote specifics will be deemed unacceptable, and therefore not considered for award. Basis of award for this contract will be to the vendor whose quotation (conforming to this solicitation) is determined to be the lowest priced, including labor, with no discussions IAW FAR 13.106-2(4)(i). (End of Clause) In accordance with FAR 13.004 -- Legal Effect of Quotations the following information is provided in regards to quotations. A quotation is not an offer and, consequently, cannot be accepted by the Government to form a binding contract. Therefore, issuance by the Government of an order in response to a supplier's quotation does not establish a contract. The order is an offer by the Government to the supplier to buy certain supplies or services upon specified terms and conditions. A contract is established when the supplier accepts the offer. When appropriate, the contracting officer may ask the supplier to indicate acceptance of an order by notification to the Government, preferably in writing. In other circumstances, the supplier may indicate acceptance by furnishing the supplies or services ordered or by proceeding with the work to the point where substantial performance has occurred. If the Government issues an order resulting from a quotation, the Government may (by written notice to the supplier, at any time before acceptance occurs) withdraw, amend, or cancel its offer. Base Access Requirement  Per Air Force Instruction 31-113, Installation commanders (or appointed representatives) may deny access and access credentials based on information obtained during identity vetting that indicates the individual may present a threat to the good order, discipline and morale of the installation, including, but not limited to the following:  There is a reasonable basis to believe the individual has submitted fraudulent information concerning his or her identity.  There is a reasonable basis to believe the individual will attempt to gain unauthorized access to classified documents, information protected by the Privacy Act, information that is proprietary in nature, or other sensitive or protected information.  There is a reasonable basis to believe the individual will unlawfully or inappropriately use an access credential outside the workplace.  There is a reasonable basis to believe, based on an individual's criminal or dishonest history, that issuance of an access credential poses an unacceptable risk to the installation/mission.  The individual has/had been barred from entry/access to a Federal installation or facility.  The individual is wanted by Federal or civil law enforcement authorities, regardless of offense or violation.  The individual has been incarcerated within the past ten years, regardless of offense/violation, unless released on proof of innocence.  The individual has any conviction for espionage, sabotage, treason, terrorism, or murder.  The individual's name appears on any Federal or State agency's -watch list or -hit list for criminal behavior or terrorist activity.  The individual has been convicted of a firearms or explosive violation within the past ten years.  The individual has been convicted of sexual assault, armed robbery, rape, child molestation, child pornography, trafficking in humans, drug possession with intent to sell or drug distribution, or is a registered sex offender.  There is a reasonable basis to believe, based on the individual's material, intentional false statement, deception, or fraud in connection with Federal or contract employment, that issuance of an access credential poses an unacceptable risk to the installation/mission.  There is a reasonable basis to believe, based on the nature or duration of the individual's alcohol abuse without evidence of substantial rehabilitation, that issuance of an access credential poses an unacceptable risk to the installation/mission.  There is a reasonable basis to believe, based on the nature or duration of the individual's illegal use of narcotics, drugs, or other controlled substances without evidence of substantial rehabilitation, that issuance of an access credential poses an unacceptable risk to the installation/mission.  A statutory or regulatory bar prevents the individual's contract employment; or would prevent Federal employment under circumstances that furnish a reasonable basis to believe that issuance of a access credential poses an unacceptable risk to the installation/mission.  Additionally, Dyess AFB has the following policies regarding traffic safety:  Seatbelts must be used at all time while vehicles are in motion  Posted speed limits must not be exceeded.  Texas Traffic Laws  Cell phones cannot be in driver's hands while vehicles are in motion, hands free devices may be used while driving  Dyess AFB personnel will strictly enforce traffic laws with random checks. Those who violate the traffic laws on Dyess AFB are liable for fines and can have their base driving privileges restricted for the following time periods of time: A. First Offense-30 Days B. Second Offense-6 Months C. Third Offense- 1 Year D. Fourth Offense- Complete loss of driving privileges Note: It is the Contractors responsibility to ensure that their employees meets/adheres to the above requirements. No consideration (e.g. additional contract performance time) will be granted by the Government if the contractor or their employees do not meet the requirements as specified above. SUBJECT: Appeal Process for Installation Access I. You have been denied access to Dyess Air Force Base due to the discovery of one or more disqualifying factor(s) identified during your background check. Air Force lnstrnction 31-113, Installation Perimeter Access, dictates Installation Commanders may deny access and access credentials based on information obtained during identity vetting. Examples of denial of installation access may include offenses ranging from a misdemeanor(s) to a felony conviction(s) or anything which may present a threat to the good order, discipline and morale of the installation. We encourage you to contact the Texas Department of Public Safety for more information pertaining to your criminal history records. 2. If you feel our office is in error•or you believe your specific circumstance warrants further review, you may appeal to the 7th Mission Support Group Commander for reconsideration of your case by submitting a memorandum of justification containing your full name, date of birth, social security number, the slate of issueance of your driver's license as well as your driver's license number directly to the Dyess Air Force Base Visitor Control Center. 3. The appeal process outlined in this memorandum does not apply to members who are currently barred from Dyess Air Force Base or any other DoD installations. lf you have any questions regarding the appeal process for installation access, please contact the Security Forces Visitor Control Center at (325) 696-8544 from 0800-1630 Monday through Friday.
