- Potential Delivery Sites (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) Solicitation number 14-236-SOL-00047 applies, and is issued as a Request for Quote (RFQ). (iii) A statement that the solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 05-76. (iv) The requirement is a set-aside for small businesses only. NAICS code 339112 applies with a Size Standard of 500 employees. The Indian Health Service intends to award a multiple year, Indefinite Delivery Indefinite Quantity (IDIQ) contract vehicle resulting from this solicitation. If the maximum quantity of 300 is reached for a line item prior to expiration of the contract, the subsequent option year(s) for the line item will not be exercised. Each contract period is for one year long. (vi) The purpose of this acquisition is to purchase the hardware and its accommodating software for preserving evidence for sexual assault examinations. All items proposed in response to this solicitation shall meet or exceed all specifications listed in the attachment A. (vii) Place of delivery: There will be various delivery sites within the United States (See attachment B). Prices are FOB Destination. All deliverables shall be received by IHS within 45 days of order. Inspection and acceptance shall be at destination. (viii) FAR 52.212-1, Instructions to Offerors-Commercial Items (Apr 2014), applies to this acquisition. (ix) FAR 52.212-2, Evaluation-Commercial Items (Jan 1999), applies to this acquisition. Addenda to the Provision -- (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: a) Technical Capability, b) Relevant Past Performance, and c) Price. See the attachment B, for more details on evaluation factors. Technical and past performance, when combined, are more important than price. (x) Offerors must include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items (May 2014), with its offer. If applicable, follow the instruction under 52.212-3 (b)(2). (xi) FAR 52.212-4, Contract Terms and Conditions-Commercial Items (May 2014), applies to this acquisition.
