Kingsbridge Road 9b-16 Bronx, NY 10468 POC: Josephine Tuitt; e-mail to: Josie.Tuitt@va.gov[Contracting Officer] DESCRIPTION: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation VA243-14-Q-1793 is issued as a request for quotes (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-76. The North American Industry Classification System (NAICS) number is 485991 and the business size standard is $15.0 million. Requestors shall state their business size on their proposal. Proposed items must be EQUAL to the items requested below. They must perform to exact specifications of the OSHA and STRICT COMPLIANCE WITH THE NEW YORK STATE DEPARTMENT OF TRANSPORTATION RULES AND REGULATIONS. Contractor SHALL be asked to prove the performance and characteristics through schematics or demonstration, to be scheduled with the Contracting Officer. All responsible sources may submit a proposal which shall be considered by the contracting officer. The Department of Veterans Affairs Medical Center, NY Harbor Healthcare has a requirement for Medical Transportation (Ambulette) Service. The contractor shall provide all labor, material, equipment, vehicles, vehicle maintenance, documentation, reports, medical personnel and supervision necessary to provide Ambulette service for beneficiaries of the NY Harbor Healthcare Medical Center's (423 E. 23rd Street -Manhattan, 179-00 Linden Blvd- Queens, 800 Poly Place- Brooklyn ) and all CBOC locations. Contractor shall furnish vehicles sufficient to provide full and complete coverage for pick-up and delivery of all veteran patients requiring transport by ambulette service to/from the medical centers and CBOC's. The provision at 52.212-1, Instructions to Offerors-Commercial applies to this acquisition and offerors must comply with all instructions contained within it. Award will be made to the lowest price, technically acceptable offer. The following additional provisions and clauses apply: 52.212-3, Offeror Representations and Certifications-Commercial Items, a completed copy of this provision shall be submitted with the offer unless ORCA registered which must be noted in quotation. 52.212-4, Contract Terms and Conditions-Commercial Items. 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, to include clauses: 4, 6, 23, 26, 27, 28, 29, 31, 38, 42, and 48.. This contract incorporates one or more clauses by reference, with the same force and effect as if they were given in full text.
