Questions and Answers, Edition #2, dated 15 September 2014 Offer Schedule, Revised 15 September 2014 Statement of Work, Revised 15 September 2014 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation Number F2F3344178A001 is hereby issued as a Request for Proposal. This combined synopsis/solicitation and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-76, effective 25 July 2014. This acquisition is 100% set-aside for Small Business Concerns. The North American Industry Classification System (NAICS) code for this project is 811310, and Standard Industrial Classification (SIC) code J078. The Size Standard for NAICS 811310 is $7.5M. The contractor shall provide all personnel, tools, equipment, materials, supervision and other items necessary to provide preventative maintenance, inspection, repair, clean and sanitize physical fitness equipment at the Fitness Centers on Hurlburt Field as defined in the Statement of Work. Work shall consist of both and Equipment Repair and Semi-Annual (2 times per yr) Preventative Maintenance. See below Contract Line Item Number(s): Contract Line Item (CLIN) 0001: Fitness Equipment Inspection, Preventative Maintenance and PM repairs Work shall be completed in accordance with the Statement of Work. Service will include, but not limited to inspections of parts for wear, adjustment impending breakdown, adjustments for wear, worn or damaged parts, cleaning, lubrication, and protection from deterioration by corrosion to ensure safe and dependable operation status of equipment at all times. Period of Performance 30 SEP 14 through 29 Sep 15. Contract Line Item (CLIN) 0002: Reimbursable Parts & Associated Labor - Not to Exceed As specified in the Statement of Work. This is a Not to Exceed CLIN. Period of Performance 30 SEP 14 through 29 Sep 15. See attached Statement of Work for description of the requirement. Delivery terms are FOB Destination to 1 SOFSS Aderholt (90517), Commando (90232) and Riptide (91007), Hurlburt Field, FL. 32544 The following provisions and clauses apply to this solicitation and will remain in full force in any resultant award: FAR 52.212-1, Instructions to Offerors - Commercial Items (Apr 2014). It is amended to read: Offeror shall submit signed and dated offer to 1 SOCONS/LGCA, Attn: Jesse Capik, 1 SOCONS, 350 Tully Street, Building 90339, Hurlburt Field FL 32544-5810, by 17 September 2014, 2:00 p.m. CST. Submit signed and dated Offer Schedule marked with solicitation number F2F3344178A001. The offeror may also submit an electronic copy of their signed and dated offer via e-mail to jesse.capik@us.af.mil with the solicitation number in the subject block. The Government reserves the right to evaluate proposals and award a contract without discussions with offerors. Offers shall be evaluated in accordance with FAR 52.212-2, Evaluation of Commercial Items (Jan 1999) which is incorporated into this Request for Proposal, with an addendum to paragraph (a) as follows: The Government will award a FFP contract to the technically-acceptable and responsible offeror with the lowest priced offer. Technical acceptability will be determined by the offeror: 1) Maintaining at the time of the requirement evaluation/award, a current registration in SAM as a Small Business for the NAICS 811310 2) Submission for the company or technicians projected to work on a subsequent contract, of training certificates, certification(s), OR examples of current/past performance information, which shows the offeror is trained/certified or has experience with, and ability to repair, inspect, and provide preventative maintenance for the brand/type equipment described in the Statement of Work Appendix 1. - For current/past performance submissions, provide at least 2 and not more than 5 examples of contracts for repair of similar brand/type of equipment. The current/past performance information must list the contract number (if a Govt contract), customer POC name and phone number, brief description of work involved (Equipment Types) and the contract value. 3) Documentation/certification for the company or technicians projected to work on a subsequent contract, of at least five years experience in the fitness equipment maintenance field. 4) Submission of the attached offer schedule with all fill-ins completed. a. Prace Evaluation - Price shall be determined by adding the following i. Annual Price for CLIN 1 (Inspection & Preventative Maintenance) ii. Govt set amount of $2,500.00 for Reimbursable Parts & Associated Labor 1. This price may be changed at award to accommodate available funding iii. Offeror provided per hour Labor price multiplied by the estimated 75 hrs anticipated as being required. 1. The number of hours may be more or less and the NTE amount in CLIN 2 will be adjusted accordingly. The offeror shall be determined technically acceptable and available for consideration of award only if minimum qualifications listed in items 1-4 above are met. *Note*: If the Offeror believes this Service is exempt from the Service Contract Act of 1965 due to the maintenance and repair nature of this requirement please refer to FAR 52.222-48 - Exemption from Application of the Service Contract Act to Contracts for Maintenance, Calibration, or Repair of Certain Equipment Certification. All prospective contractors must have CAGE codes and DUNS numbers, as well as, be registered in the System for Award Management (SAM) website at https://www.sam.gov/portal/public/SAM/ (formerly CCR) to be eligible for award. Processing time for registration in SAM may take five business days or more. Contractors may obtain information on registration and annual confirmation requirements via the SAM website or by calling 866-606-8220 or 334-206-7828 for international calls. AFFARS 5352.201-9101 OMBUDSMAN (Apr 2014) (a) An ombudsman has been appointed to hear and facilitate the resolution of concerns from offerors, potential offerors, and others for this acquisition. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. The existence of the ombudsman does not affect the authority of the program manager, contracting officer, or source selection official. Further, the ombudsman does not participate in the evaluation of proposals, the source selection process, or the adjudication of protests or formal contract disputes. The ombudsman may refer the interested party to another official who can resolve the concern. (b) Before consulting with an ombudsman, interested parties must first address their concerns, issues, disagreements, and/or recommendations to the contracting officer for resolution. Consulting an ombudsman does not alter or postpone the timelines for any other processes (e.g., agency level bid protests, GAO bid protests, requests for debriefings, employee-employer actions, contests of OMB Circular A-76 competition performance decisions).
