Description

PRE-SOLICITATION NOTICE X-RAY IMAGING SERVICES The VA Central Iowa Health Care System (VACIHCS) intends to issue a solicitation for x-ray imaging services to be performed at the contractor's site (to be located in the city limits of Fort Dodge, IA) tech. services. a.Routine x-ray imaging The Contractor shall furnish all equipment, facility, and technical personnel services to provide routine x-ray images for eligible veteran beneficiaries as referred by the Fort Dodge Iowa Community Based Outpatient Clinic (CBOC), Fort Dodge, Iowa a part of VAMC Des Moines, Iowa. b.The Contractor's facility must be located within the city limits of the CBOC located at 2419 2nd Ave North, Fort Dodge, IA 50510, in order to allow patients to receive radiology services in a timely, safe and efficient manner without compromising patient care or paying excessive for the Veteran travel pay. Contract services will be provided on an as needed basis. Approximately 50 patients per month will be referred. c.The services to be performed by the contractor will be performed in accordance with radiology protocols of the VA facility. d.Eligible veteran beneficiaries will be directly referred from the Fort Dodge CBOC to the Contractor. Patients will be identified as an eligible veteran beneficiary through presentation of a Veterans Universal Access ID Card a government/state issued identification. e.Personnel assigned by the Contractor to perform the services covered by this contract shall be certified by the American Registry of Radiologic Technologists. f.The qualifications of such personnel shall also be subject to review by the VA Chief of Staff and approval by the VA Imaging Chief. g.The contract will have a 12-month period of performance with four one-year long option periods. Services will be required beginning approximately October 1, 2014. The North American Industry Classification System (NAICS) Code is 621512 (Size Standard: $35.5 million). The Government anticipates awarding a fixed-price indefinite delivery-indefinite quantity (pricing to be based on a percentage of Nebraska Centers for Medicare and Medicaid Services (CMS) Physician Fee Schedule and Clinical Diagnostic Laboratory Fee Schedule). The request for proposal is anticipated to be released on or about 9/17/2014, on the Federal Business Opportunities website: http://www.fbo.gov. Prospective offerors should not expect notification of issuance of amendments other than via the Federal Business Opportunities website and are responsible for obtaining all amendments as posted. Prior to submission of a proposal, offerors are expected to read the solicitation documentation in its entirety to fully understand the scope of work, terms, and conditions of the requirement(s). Offerors who are unable to access or open uploaded attachments may contact the Contracting Officer to obtain copies. All questions pertaining to the anticipated solicitation shall be in writing only and submitted to: gary.hausdorf@va.gov.
